Dear List:

I have a question for other Colorado providers.

I just heard from Nursing that we are supposed to include a diagnosis on

every telephone order, including therapy orders, in LTC. I've not heard of

this before. Seems most problematic when we request orders based on screen

or Nursing/family concerns and of course have not yet evaluated.

Has anyone else heard this? Rumor is that other facilities may have been

cited for this during state survey. Can any of you confirm or refute this

rumor? Is there a reference that you are aware of?

We do include treatment and medical diagnoses as appropriate on all Medicare

certifications (98-99% of our residents, with 1-2% Medicaid or other).
