Insurance Formulary - Ophthalmic steroids

I have to voice my annoyance at how insurance formularies are developed. Most formulary teams have decided that a mixed medication (2 meds produced into one product) are more expensive than the individual products. They maintain this assumption even if a generic version of the drug in question is available. Case in point:Wellcare Georgia will not pay for tobramycin/dexamethasone generic or name brand Tobradex yet tobramycin ophthalmic and dexamethasone ophthalmic are available individually on their formulary. After years of being told not to prescribe steroids for the eyes due to multiple complications that come from long term use, I have a real problem with prescribing dexamethasone for a patient that needs the compound medicine to treat a serious eye injury related infection. The really crazy thing about this formulary decision is it goes against what most of us have always been taught since the beginnings of medical school. Again, financial benefit to the insurer, is being put above patient standard of care. Comments???? Dr. Beth Sullivan, DO
