Taking comments on return to work / school forms.

Just got a patient that was out with the Flu last week, gave her my

standard; able to return to work on such date, please excuse for medical

reasons from this date to above date.

Today she comes with a document from her employer stating that she needs

a letter detailing diagnosis, illness length and certifies that she she

is able to return to work with no restrictions nor problems, written in

such format as they like.

I told her that if they required a form they should provide me with such

form. I also pointed out that I do not do workman's comp cases.

What to do?
