As for 's current situation it may be similar to what happened to me about 1.5 years prior to my tx. I too noted weight gain and had to buy new pants two sizes larger. I also felt full after just starting to eat. Nausea was a constant companion, along with gut aches and headaches. Finally I decided to check myself and found that my liver had enlarged significantly, down to my belly button. I made an appointment to see my Hepatologist and she confirmed this. No ascites was present. If there is a lot of ascites, especially later on when muscle wasting has also occurred, it can look very obvious. Guys take on a shape similar to being pregnant. There is a specific examanation that docs do to determine if there is ascites; it's called shifting dullness. First you exam the abdomen with the patient lying flat on their backs and then on their sides, tapping with your fingers on the belly and listening to hear a different sound when you're over fluid or over the bowel without fluid in the way. Sometimes, if there is a significant amount of fluid, and the patient isn't fat, you can see the fluid move when the patient rolls from side to side on their backs. But you need to know what you're looking for. I think even with all the other reasons you've given for feeling out of sorts lately, he should be seen, perhaps by his GP first who can test for an enlarged liver or the presence of ascites and order an US if s/he thinks either of these things is going on.
