Following is information on the program that was received today from Ted Granger, President of United Way of Florida, about how individuals can access an enrollment form. More detailed information and application forms in several languages are available on the KidCare website at http://www.floridakidcare.org/. This insurance program is an affordable way for children to receive quality healthcare who may not currently be covered. We ask that you please pass this information along to your employees, clients, etc. who may benefit from this program. Prepare to Enroll in KidCare in January January is open enrollment for Florida KidCare. KidCare is affordable, government-subsided, quality health care insurance for children whose parents meet the income eligibility threshold and do not have access to private insurance through their workplace. For more information and to request an application in Broward County call (954) INSURES (467-8737) or outside Broward County call 305-468-KIDS (5437). The enrollment period ends on Jan. 30. Any applications received after that date will be rejected. To the relief of many, changes made during the special session of the Florida Legislature in December made the application process simpler, reducing multiple income verification requirements. Now only one of the following three documents is required to prove income: A completed federal income tax 1040 form A W2 form A pay stub Enrollment periods have been reduced to only twice a year, so don't miss this important opportunity to obtain health insurance for your child. (This message courtesy of Modesto Abety, President & CEO, The Children's Trust) KidCare Application Now Available The limited open enrollment means parents have just one month to pickup, fill out and mail in your application. The state is using a new form that will ONLY be made available on Dec. 30, 2004. Do NOT use the old form. Pick up an application: The application is available throughout South Florida. Check a list of local organizations for the one nearest you. Download an application in English, Spanish or Creole on the KidCare website: http://www.floridakidcare.org/. In Broward County you can call (954) INSURES (467-8737), or statewide you can call 305-468-KIDS (5437) or throughout January 2005 to request that an application be mailed to your home. Keep in mind it may take few days to get the application so you might be better off downloading it from the Web. Applications must be received by the January 30 deadline.
