The idea of Near Miss Reporting is to record, investigate and analyse all incidents with the potentiality of resulting in serious injuries. A detailed analysis of all such reports recieved gives us a clear idea of the path or trend of unsafe acts/practices etc. prevailing in the site. This gives us a direction to the site safety efforts and helps immensely in controlling accidents. We, in our site encourage workmen of all categories to report on Near Miss Incidents. These reports are then communicated to all of the work force in Tool box talks and the reporter appreciated with a safety gift coupon on the spot. An analysis to identify the root causes is done on a monthly basis and actions are taken accordingly. Sincerely hope this form would be useful to you. In fact I should have written about the abbrevations in the first mail itself. It was an overlook which caused confusion to some of our friends. The abbrevations used in the format are: HiPo NM: High Potential Near Miss Incident - Which is self explanatory. SC: Substandard Condition SP: Substandard Practise H, M, L : High, Medium, Low Hope this will clear all your doubts.
