Guest guest
I think people are expecting too much from the meaningful use dollar….. Practice Fusion may still get you MU dollars….and it may not…..But that is pretty much the case for any of the EMRs…. No, that's not true. You should get the MU$ if you use a fully certified EMR. If you don't then it's because of something else not related to the EMR functionality itself. But let’s ask what it will cost you to use Practice Fusion vs other EMRs…Just recently I saw figures of some EMRs that will cost anywhere from 5000-25000 to just get started and then have monthly costs…..I don’t know how true it is… So say you get the MU dollars with the other EMRs…..then you basically make up maybe what you already paid for in the EMR…..any maybe not even all of it…. Whereas with Practice Fusion, you didn’t pay anything to begin with and you still may get the money from MU…..it offers a possible “win—win” situation whereas the other EMRs may have a “lose—lose” proposition possibility (lose on paying for an EMR and lose out on MU dollars). Again, this only works if you consider your time is worthless. There is a significant labour involved in entering patient data into an EMR, and a learning curve associated with using it. And that would be wasted if you had to move to a different one later on. As it currently stands, PF will *NOT* get you your MU$. They have to get fully certified, or, you have to get another EMR that cover its deficiencies. Just my thoughts…. BTW, link on the blog does not seem to be valid anymore Which blog? I pasted directly from those blogs today.-- Graham Chiuhttp://www.compkarori.co.nz:8090/ Synapse - the use from anywhere EMR.

NO EMR has yet to be established as ‘compliant’ with being a fully Certified EMR from the government…….. And that would be wasted if you had to move to a different one later on. Believe me, THERE is NO EMR presently that works faster than a doctor Noting on pen and paper because each doctor has their own style…I Have been exposed to a plethora of EMRs…..they all have their faults. As it currently stands, PF will *NOT* get you your MU$. They have to get fully certified, or, you have to get another EMR that cover its deficiencies. How do you know this? Are you the government/medicare agency That is certifying? Can you give me a list of EMRs that are fully certified And assured of getting MUs? Are you saying the EMR you are using is Fully certified and will guarantee getting the money? How much did it Cost you….. Just my thoughts…. Guest guest
OK, I missed your signature, as perhaps many others did.Perhaps, or perhaps it was just you. I did too So, let me clarify this. When I was corresponding with you a couple of years ago trying to help you with your PDF forms, you were completely unaware that not only was I a specialist IMP (for 17 years) physician, but I was also a developer of EMRs?? And all those posts where and I talked about developing EMRs were not read by you?-- Graham Chiuhttp://www.compkarori.co.nz:8090/ Synapse - the use from anywhere EMR. Guest guest
Graham,I think the point that Sangeetha and I were making is that your signature is not a very prominent place to notice that you have something to do with Synapse. Plus, not everyone on the listserve has been around long enough to read your previous posts. For the sake of the newest listserv members, and to ensure complete transparency, I think it would be better to make any potential conflicts of interest more explicit. Guest guest
No I am well aware that you are very interested in Emrs, I also remember you talking abt the cost of certification, But the signature barely registers, also I didn't thin k there was any conflict mostly because I was/am still uder the impression , That your veographical market was entirely different. Does alex have an emr product too?.I know only of oxbow as the other emr developer/physician on the list. So it seemed/seems to me that yolu are kindly observer.. Guest guest
A few signature do register..esp the ones that go " solodoc 5 yrs, never never work. For a hosp again " . I know alex was into emrs but must havejoined the listserv late didn't know he had a complete saleable product. There,s a difference .no?. I think its also a function of your dual roles as docs and. And developers. You don't want the second role to overshadow the first on this listserv.. and righgtfully you have been downplaying it. But that also means the only product that a physician/developer has come rightout and said " hey I have a product, that you can get started on tomorrow " is oxbow. Everybody else in my eyes is " working on it " .maybe this kind of to and fro is an opportunity to clarify that? Guest guest
I think this bickering back and forth has become really unproductive. We are all now aware that Graham has developed his own EMR. I saw the signature; I didn’t realize he was saying Synapse was all his. Whatever. He wasn’t dissing Practice Fusion and promoting Synapse. In fact, I believe Graham is in New Zealand and none of this Meaningful Use stuff even applies to him. But he seems to have taking an interest in it all the same and is a good source of knowledge, whether an EMR developer or not. And Graham and I have bumped head. I don’t always appreciate his opinions but I listen anyway. I agree with what he was basically saying. Many EMRs are getting “certified” but are not the complete product. So for the physician to be certified they are going to have to find solutions that are certified to the missing pieces. It can be done but I don’t know if everyone is aware of that. My hospital is promoting Allscripts right now with “great discounted pricing for all of our doctors and guaranteed certified for MU”. I keep bringing up, great, but is anyone going to help the doctor make the MU cut? There are pretty much pushing doctors to buy Allscripts so they can get MU money. They have no knowledge of how difficult it will probably be to start using an EMR and meet MU criteria all in 1-2 years. I don’t know if Allscripts is certified as the complete package. I don’t know who is and who isn’t. I know mine is and I am relieved as I will have less things to figure out and solve. But other people may have that all worked out already. Anyway, Graham, may not have not been as open and upfront as some of us might wish but what he was discussing wasn’t even really a conflict of interest for him. Let’s learn and move on please. Guest guest
I don't have an EHR yet but I just went thru the process of certification for PCMH which is much the same as for MU. Much of what you need to do to be certified is to demonstrate that you are able to do a data task, ie track f/u on abnormal labs or PAPs. Not having an ER, I developed my own methods of tracking things. Having a computerized way of doing this is much easier, and really, that is all the EHR is about. It's making tracking of various aspects of health care data do-able, not so labor intensive. For ex, I can pull up a list of all my pts with DM via ICD-9 codes on my billing soft ware. That is something I do in an electronic way without an EHR. Since I don't have EHR, however, I now have to manually go thru the charts on all those pts to review their A1Cs to see if they are current, good/bad, other. With an EHR, presumably, I could pull all that data on A1Cs electronically. And they work on the pts' whose A1Cs are missing or below goal. I'm probably just saying what everyone already knows but there it is. Having an EHR per se doesn't give a practice automatic MU. You have to input data and use it. And the MU bit is all about demonstrating thru screen shots and other methods that you regularly collect and input needed data and that you make us of it. Certified EHRs are ones that allow the user the functionality to do certain tasks. But to the get payment for MU, one has to make use of the tools. And that is up to the user. Maybe this is common knowledge but the talk in these emails seems to suggest that having a certain EHR will automatically grant MU status. And that is not so. IT's up to the user to make that happen. It's not a given just because you buy a certain EHR. Without an EHR's electronic tools, however, accomplishing some of the MU standards is too inefficient to be practical. But many things are just good medical practice--keeping an up to date Problem List and Medication List. Updated data on Allergies. etc. How many of us check the Med List q visit and make sure it's up to date all the time ? That's the hard part and that's what MU is all about. It's tedious and annoying, and not what we went to medical school for, to be data clerks, but staying up to date on our pts is a part of good medical care. EHR's don't make that happen, we do. It's not a panacea, simply a tool. OK. End of lecture. Sorry for boring everyone.
Guest guest
EllenI think you've missed the point. MU is not about how well you treat your patients. MU is showing that you can use EMR according to a number of criteria. All your patients could the worst outcomes in the state .. it's not relevant to MU .. at least for this round. The govt wants eligible physicians to be able to demonstrate that they can upskill to using EMRs. Outcomes will follow eventually. Guest guest
As the meaningful use criteria are more or less set and dollars are set to start rolling in this year, just curious if any Practice Fusion users are planning to apply for meaningful use incentive dollars. Some had mentioned on this board that a free EMR like Practice Fusion may not be able to deliver on meaningful use criteria, though without any substantial facts or specifics detailed. Also, any Practice Fusion users have issues with service downtime? That would be the main issue I would worry about with a hosted EMR. Though at least, according to the PF website, they are tops in customer satisfaction. It is tough to beat free when one is considering a low overhead practice model.
