But just a couple of things I wanted to say in response. Forgive me if you already explained these things and I didn't catch it.There are many psychiatric diagnoses involve genetic factors, this is a fact. According to Dr. J bout 17% of our members here are officially diagnosed as OCD, with OCD there is a genetic link I believe. While in the general population OCD is diagnosed in only 2 % of the population. Not that I am saying that misophonia is OCD. But given this higher percentage of comorbidity with a psychiatric (which is also neurological problem) like OCD here, suggests (I think) there must be a neuro something or other, happening with misophonia.In your example with tuning fork, in such a case, had the child heard the tuning fork before? In which case it is quite obviously a conditioned response. I am talking about visual triggers, unrelated to the sounds, unrelated to areas or places or body parts where sounds come from. My mother twirls her feel around, it makes no sound, I have no problems with the sounds of footsteps or any other foot related sounds, yet this is a very strong trigger for me, even though I can manage it by just looking away, which then stops reaction immediately. If misophonia is a generalised (over)conditoned reaction to sound, then other stimuli can then act as triggers. Some audiosensitive patients will rear away when an otologist approaches them with a tuning fork before it is even struck. Unfortunately, this is usually interpreted to mean that the person is a hopeless neurotic, and should be seen by a psychiatrist instead. The four comorbid conditions mentioned below as examples of neurological conditions are in fact psychiatric diagnoses. Whilst I think it reasonable to suppose that the brains of such patient are not working properly, I think it is fair to say that no one has a clue as exactly what is wrong with the brains. Even when structural brain abnormalities are found, it is far from clear that these are the cause rather than result of the psychiatric or psychological symptoms. Comorbidity is the rule rather than exception in psychiatry, and there is no solid base of understanding from which one can bootstrap from known to unknown mechanisms. In otology, by contrast, there is a solid base of understanding which can be used as a supply depot for explaining many supposedly psychiatric conditions (eg agoraphobia). As for people without auditory problems on this site, it is analagous to the situation where someone hears unexplained sounds. I have just been checking the excellent Wikipedia article on OCD, and can see nothing there which shows that it is due to primary brain damage. This contrast with say multiple sclerosis or Parkinson's, where there are satisfactory neurological models. The fact that OCD is associated with above average IQ is surely evidence that is is due to pathological (over)thinking. So I do not think that OCD helps at all in establishing a neurological or brain basis for misophonia. The aversion to sight of a tuning fork referred to adults. I was intrigued by your reference to twirling feet(?). I love to look at it, and love love love to listen to it. I don't have the "typical" symptoms most people would associate with OCD, although I can be very obsessive about certain things. My IQ is unremarkably average. My psychiatrist is currently trying a treatment plan for me with misophonia as being a function of OCD, I do not have a diagnosis, and he is not married to the idea of this being OCD, but it is an obvious starting point for a psychiatrist. He can only diagnosis me his patient, not an entire group on the internet. If he wants me to see an audiologist or otoneurologists, I will comply, if I can find a way to afford it.But it's great to hear different theories and points of view. OCD is surely a result of the brain overfunctioning more than underfunctioning. I don't know if patients aversive to tuning forks had seen them before, but I think anyone going to an ENT clinic seeing the doctor waving a fork around might assume that some loud or unpleasant sound might come out of it. I seemed to have passed even though I only heard the high pitched sounds. I have been starting to expirienceing tinnitus when I wear my eaplugs. I still can't hear people so well. Would misophonia effect my hearing? But was lucky enough to be sent to good schools. OCD is surely a result of the brain overfunctioning more than underfunctioning.
