The Family Caregiver Journal is a tool for families to plan, organize or reflect about their caregiving experience. The journal can be used to communicate with family members, physicians, health care provider or helpers. It can help families make decisions, change ways care is provided, or find help, such as personal care, meals or chore services, that may be available in the community.
