Guest guest Posted August 6, 2010 Report Share Posted August 6, 2010 So I have a new patient who has just been commenced on Warfarin for AF. It is intermittant. She has had a TIA which was the diagnostic event. Was very reluctant to take the warfarin but has started in the last few weeks. However the warfarin nurse has started very small doses rather than the usual loading doses and still 2 weeks later her INR is only 1.1. I am aiming to reduce her epsiodes of AF with a view of her being able to have a repeat 24 hour tape in 6 months. If the AF has then resolved she can decide if she wants to stop her warfarin with her GP support. The whole problem is compounded by stress and anxiety and since commencing the warfarin she is not sleeping. BP is OK. Pulse on examination is 75 reg. She is 70. Very fit, great diet, yoga teacher! I am really struggling with her prescription. I came up with: Tilia Cytisus Leonorus but then I started checking the cytisus and leonorus and they seem to reduce coagulation. Which is actually what she needs at the moment being that her INR is going no where near the target 2.5. She is having weekly INR's at the moment so at least she is being closely monitored. But I dunno....... Written to GP. Help!
