Angry acne

UKHerbal-listDear All

I have a patient, F, early 30s, with constant acne that reddens and gets sore

just before her period. Periods are regular but very heavy. GP investigations

found no pathology but her uterus is very large. She was born with hemangioma

of the lower limbs for which she wears support stockings and the Pill caused

problems with pain there. She has a history of depression which is worse in the

winter and been on a variety of antidepressants. No medication at present and

just finished 'successful' counselling. Very busy with work, compliance poor,

unwilling to take much for it.

Some improvement with Dandelion root, Centella, Ladies Mantle, Echinacea,

Hawthorn, Liquorice (5 ? reductase) (didn't want to cause massive worsening) but

pimples became angrier again before periods. Compliance spasmodic so gave Vitex

(10gtt am) only to help compliance, with same results. Clay packs and

baptisia/witchhazel lotion help. Recommended diet, zinc, B vitamins and omega

3. Am unsure whether high dose of latter is wise re: her hemangioma?? I warned

her about time scales and exacerbation and am encouraging her to make lifestyle

and dietary changes but wonder if there is something else that could make her

more comfortable which would motivate her to make small changes?

Which is the best supplier of calendula cream? I want it to be good and

wholesome, to look like calendula (ie. orange colour), not too slack (so that I

can stir things into it), and have the ingredients listed on the pot. I want to

buy it in 500g or 1kg amounts.

could you expand on " Hemangioma of the lower limbs " ? My understanding

of hemangiomas was that they are discrete benign childhood vascular

related tumours that tend to disappear with adulthood. Does she still

have multiple ones or am I missing something I should know ?? ;-)

(nice prescription, I'd have squeezed in some galium for more

" outspoken " lymph drainage and I'm wondering about Paeony,)

Hi Gail,

The dose of Vitex might be too small; 3 1/2 mls per week is quite low. How

about increasing to 5-7ml per week?

why has she come to you if she doesnt want to take much for it??!!

What about Berberis aquifolium, which is specific for skin problems and a nice

bitter which might cheer her up. I gather it shouldn't be used with liquorice

though as the liquorice will nullify its effects.

Hi Gail,

I find Stellaria really useful in an Rx to calm angry skin eruptions and have

used it for acne many times.

Hi

Vitex is one of those herbs that is posited to have different effects at

high and low doses. (Ruth trickey)

I often use 12 drops mane as a dose for PMT and/or dysmenorrhea and find

it a very useful dose (As I expect you know it's reckoned to be

optimally absorbed in the morning due to the increased activity of the

pituitary then)

I love Berb aq for skin - it's great isn't it.

I've not heard the idea about not using it with Licorice where does it

come from do you know? (I thought most chinese prescriptions used

licorice, and they often have a lot of herbs in...) (I don't use a lot

of licorice as a rule, but have been thinking of using it more so would

like to know more)

I couldn't remember offhand where I'd got it from but a quick Google of those

terms came up with this exact point, which comes from:

Duke. J. A. and Ayensu. E. S. Medicinal Plants of China Reference Publications,

Inc. 1985 ISBN 0-917256-20-4.

Which I haven't read, but it might be worth checking out.

iuve never heard of problems with berb aqui and liquorice - do you have a

reference for that?

I know a lot of interactions are theoretical rather than evidenced -

there's an interesting discussion re carduus and statins on the members

forum - and a very interesting contribution from Jonathon Treasure made

this point - so I'm a bit wary....

Hi Sally

The gist of the quote - 'It should not be used with Glycyrrhiza species

(Liquorice) because this nullifies the effects of the berberine' - appears in

various places if you do a Google search, always attributed to that source.

I have no experience of this myself - it may well prove to be theoretical - but

just thought I'd point it out.

Hi thanks for your suggestions. Berberis aquifolium was on my list and I'm not

sure why I did not use it. I think I was worried that it might be too strong

for her initially. It cooled one patient down so much he complained of

shivering. Yes galium or stellaria are excellent possibilities. Hadn't even

thought of anemone. Liquorice does seem to have more than its fair share of

theoretical problems, doesn't it. I am aware that the dose of vitex varies

depending on what you are trying to do but do find getting the right dose quite

tricky which was why I avoided its use in the first place. I wonder about

serenoa?

The patient says her right leg hurt in the mornings as a child. When it is cold

right leg is painful and when she took the pill she got pain and some slight

swelling there which led to MRI scan 2009 which revealed malformed veins in that

leg and on GP advice she has used support stockings ever since. Still unsure if

high dose omegas would be suitable in this case.
