A few miscellaneous questions

Hi,

1. Family history is positive for s/l ____ *alfa 1* antitrypsin

deficiency in his maternal grandmother. Cannot document.

2. Pt. does have a dry area of erythema and scabbing with a few open

lesions. No _____s/l *frank or flank* pus pockets are identified

3. Sleep study results showed a respiratory disturbance *index of 39

per hour* but there was no significant *arterial desaturation.* Is this

correct? Also need verification on format.
