I just made the attached document to help us keep track of Mom's patch

placement. It is a word doc. Feel free to just click on the table to

change the dates for your own use. We are placing a copy of the this in her

emergency " Care Book " . I'm also willing to upload what I'm using for that

if anyone wants to look at it or take it and modify it to make it their own.

Gladys, THANK YOU for sharing the chart! I'm sure many will be able

to benefit from it... If you're really willing, go ahead and upload the

Care Book too -- if you need to make it 'generic' and don't have the

time to do so, you can forward to me and I'll do it. Whatever is

easier - but I bet others can utilize your Care Book too!
