This is to update you to the following:

Application forms regarding waivers from corporate practice restrictions have been updated by the OOP. The file is attached and you will be able to clearly identify the changes highlighted in yellow.

Some of the changes include:

Allowing the Board member to identify a "preferred" address, phone number and email, which could be the home, business or entity information. This change is on Form CE-1 submitted by the officer, trustee, director and on page 3 of the application (Form CE), where the entity indicates the name and preferred contact information for each director, officer and trustee. In the event there are questions about information provided by the director, officer or trustee, the OOP will use the preferred information to seek clarification.

The updated forms and instructions are on the OOP website and attached here in case an entity needs to submit new or additional information.
