Please refer to the tool kit sent previously and located in the members only section of AATA. In response to the earlier discussion: Art Therapy is a reimbursable service and individual’s with a license should first complete the NPI form to apply. Here is the process that art therapists should follow to registered as an individual provider of Medicare. 1. An individual needs to apply for an NPI number. The web address to the NPPES Downloadable File can be found at: http://nppes.viva-it.com/NPI_Files.html. (An underscore follows "NPI".) Those individuals planning to access the downloadable file should refer to the information provided on this page before attempting to download the file. As indicated under the 'Facts about the Downloadable File', the file is intended to be downloaded by individuals with the requisite technical expertise. To Enroll as Individual: 1) Complete Form CMS-855I (Physicians and Non-Physician Practitioners): http://www.cms.gov/CMSforms/downloads/cms855i.pdf (NPI) APPLICATION/UPDATE FORM File Format: PDF/Adobe Acrobat May 30, 2007 ... NATIONAL PROVIDER IDENTIFIER (NPI) APPLICATION/UPDATE FORM. Please PRINT or TYPE all information so it is legible. You can expand the entire list by clicking the "Open All" link above. Expand the code list to view the more detailed codes. Use your browser's find feature (Ctrl-F) after expansion to search for values. Taxonomy codes are self-selected. Choose the code that best identifies you as a provider. Once the applicant is under D. Taxonomy Code the individual would list code 22 which is for ‘art therapy’. See below.
