Creative Arts Therapist Limited permit

Hi Community, I received my degree, MSc in Art Therapy, in the U.K. I am wondering what steps should I take in order to get the equivalence of my degree in New York State and become eligible for the Limited permit.In Creative Arts Therapist Licensing Application Packet in Form 2 "Certification of Professional Education", in Part B,regarding all other programs, "An official transcript or marksheet giving courses completed by year and grades and a syllabus of the course of studies..." is required. Is this the Form that I have to complete? I would greatly appreciate any advise on how to proceed. Despina Myriokefalitaki - Zografos
