Maintains documentation on each resident on assigned floors.Responsibilities:Welcomes and orients new residents to the facility and visits all newly assigned resident on a daily basis., Interviews, evaluates and assesses each newly assigned resident. Designs a program specific to resident's needs and physical condition. Encourages best use of resident's free time. Transport resident to and from activities. Transport residents to programs and during community trips. Initiates and maintains progress notes, MDS + and CCP on each resident on assigned floors. Provide documentation of a review with the residents of their program participation and any changes as needed. Maintains professional attitude and appearance. Performs other related duties as assigned.Qualifications:BA/Bs in Music therapy, Masters degree preferred.
