What should I say tomorrow?

I all, I have an apt. with the spine center tomorrow and need to know " what " I

should tell the Dr. about my symptoms?

I dont know if or what of these are or could be related to my TCS:

Severe Headaches

Blatter frequency & urgency

neck pain

back pain, upper & lower

Saitica pain down leg for 3 months

memory loss, forgetfulness

Hearing loss...(cant hear when there is background noise)

Of course, scolosis and leg & foot deformity

My neuro & pcp put me on:

Topamax

Noratripaline

Methocarbanal

Naproxin

None of course are helping.....

Any Advice for tomorrow would be GREATLY appreciated!

I want to make sure I am asking everything I need!
