Was told to take a breath to clear out the spacer then puff the meds and breathe 5-6 times into the spacer to get all the meds out of it. Works for me. It's intended to keep all the medicine from landing on the back of your throat. Actually I do usually inhale several times - works better that way. I just wasn't told to do that. My doctor wants me to use it with the Symbicort and Q-var inhalers as well. This is a link to one very similar to what I've used in the past. The MDI (rescue puffer) is inserted in the back, you exhale all the way and then just as you begin to inhale you discharge the MDI.
