No this isn’t addressed on the forms, but it can easily be added 2) Is there anything out of date or is it one of those classics? Nothing appears to out of date – with the except of what is address in the first question here in this email. If I were revising this publication or creating a wish list for revisions: 1) I would make it a loose leaf so that the forms can be easily photocopied 2) I would add the issue about the need for RDs to participate in CEUs to make their learning goals 3) I would consider placing it on a CD in word format so that it can be modified to suit each dietetic professional or facility. As it stands now I am going to separate the book and put it into loose leaf format. I have considered scanning each form, but I think that might be more hassle than it’s worth. To modify for individualization, it would be just as easy to create my own form based on the information in each form.
