Giving medical advice on phone without compensation

Does anyone have any ideas for getting compensation for telephone advice? I would love to add telephone visits/consults to my practice as some patient complain that they cannot always make their clinic visits for fear of job retaliation. They call in to try to get me to prescribe antibiotics for "sinus infections" which I refuse to do. Any thoughts? While on the one hand I don't really feel I can afford staff right now I also don't feel I can take on more patients without hiring staff.
