am inclined not  to sweat the small stuffIf someone wants a form they bring it to youI f not,  then " seen today Cleared for all activities " and sign your name.Jean

Need some advice.

 

Attached is my new (and still needing revision) employment/sports physical form.  As you can see, at the bottom of the second page is a declaration that the person is cleared or not cleared, or cleared with rehab.  Should I provide this entire form to the employer?  Or do I need another form that doesn't give such detailed health information but simply states the clearance or non-clearance? 

 

I went to a day care and provided 16 physicals yesterday (marketing my practice).  Didn't think about this privacy issue until I was inputting them into my EMR today.
