But now that I look at it and with some comments from patients, I guess it is a little confusing. Any critique on the example of my Statement below -- or post examples of yours that you think are better or good. In the example below, I thought the example was OK -- Claim # with DOS. Then lines of what code/service and the charges. Then a list of the XYZ Health Payment -- the insurance company's payment. Then a payment of what the patient owes. The previous example was what I can see on my EMR. Attached in pdf is the version that gets sent out. It's better. The main thing I could see changing -- the description for the labs, visits, etc. I can update these phrases in my EMR and they will print out how I update them. For example, patients may not know what HPV typing is. I'll need to think about how to update these and what they should say. For example, the Draw -- is the fee charged for drawing -- never get paid that, but it is a cpt charge. Also, below is an example of what it says on the statement when the insurance adjusts off a certain amount. I wonder if this phrase should be stated differently. National Foundation Life Ins Adjustment should probably be phrased differently. Suggestions?
