Likely a low impact walking treadmill or bike. I would set up monthly debit. I would hope to partner with moble phone and exercise equipment makers to track my patients exercise and weight. Expand the technology to gather basic health information daily, feeling fine, no shortness of breath or chest pain, weight stable, no pain, happy, accuchecks normal, etc then transmit data into database with appropriate filters and alarms for outreach and same day appointments. I haven't got my arms quite around it yet, but I do think there is a business plan in the future. And why not call it an ACO. Who could be more accountable than PCP with daily contact to patients who want better care and coordination. Throw in dietary counseling and grocery shopping assistance, counseling for depressin and anxiety. Who knows but at $13000/year for a FAMILY INSURANCE PLAN we certainly could advocate for more actual patient services that improve health. " Kathleen, I think the big point was they are willing to have lots of input from many stakeholders. The question remains, those with limited resources will have the toughest fight to be heard and entrusted more responsibility. I think they know the patient should be central and the doctor a little off center ( no pun intended). Hospital CEO just mentioned millions in reserve for employing doctors to collect rewards of ACO's and shared payments. Wish I could employ the hospital.
