Reminder: CDR Registration Fee due by Aug 31, 2007

CDR registration maintenance fee notices were mailed

to RDs or DTRs for the 2007-2008 year in June 2007.

PAYMENT POSTMARK DEADLINE DATE

All eligible RDs and DTRs must pay their registration

maintenance fee by the postmark deadline date of

August 31, 2007.

The annual CDR registration maintenance is $50.00.

Please see below for the four (4) methods available to

pay the fee.

1. MAIL

Send your $50.00 payment and CDR registration

maintenance fee notice to:

NOTE: The CDR registration fee payment address has

changed. Please use the address above or the courtesy

reply envelope which was enclosed with your 2007-2008

fee notice.

Make your check payable to CDR and include your ID

number.

2. PAY ON-LINE

Go to:

http://www.cdrnet.org

(The system is not available from midnight to 6 a.m.

Central).

Click On-Line Services: Address Change, Fee Payment,

Learning Plans, Activity Log.

You’ll need your registration ID number and Web

Password.

Your Web password was printed on your 2007-2008 CDR

fee notice.

On your Profile Page, go to the top and click on CDR

Credentialing.

The next page, click on CDR Payments.

3. CALL

Call 800/877-1600, ext. 5500.

You can contact CDR 8:00 a.m. to 5:00 p.m. (Central)

and pay your CDR fee by credit card -- American

Express, Discover, VISA or MasterCard.

4. FAX

Send your completed CDR fee notice to 312/899-4772.

You must include the amount to be charged, your credit

card number, month/year expiration date and signature

of the credit card holder.

Please keep a copy of your fax and transmission

verification for your records.

LATE FEE

September 1, 2007 to December 31, 2007, the CDR fee is

$75.00 ($50.00 for the annual CDR fee plus the $25.00

late fee).

A $25.00 late fee is accessed for all payments

received after the August 31, 2007 payment postmark

deadline date.

TERMINATION OF REGISTERED STATUS

CDR registration status will be terminated if the CDR

registration fee is NOT paid on or before December 31,

2007.

Once registration status has been terminated,

individuals must successfully complete the

Registration Examination for Dietitians or the

Registration Examination for Dietetic Technicians to

reinstate registration.
