The Office of the National Coordinator for Health IT (ONC) in the U.S. Department of Health and Human Services (HHS) would like to learn more about the implementation and use of health IT in your organization or practice. We invite you to share your story of the journey to and through adopting Health IT, and what differences adoption has made in your patient care, operations, or other important

aspects of your work. Our goal is to identify stories about adoption, implementation, and outcomes as well as models of leadership in the implementation decision and execution process. We welcome “lessons learned†that offer constructive suggestions as well as “success stories,†although we cannot spotlight specific health IT products. These examples may be featured on our web site (http://healthit.hhs.gov/hitjourney) or incorporated into other material. If your story were selected we would get back to you to verify permission for its use. Recognize that this

is only the first of several steps in this process and there is no guarantee that your story will be featured.
