good morning,

the tfc's verify all benefits for each patient, both medical and pharmacy. yes, the tfc's meet w/ each pt during the evaluation and go over potential medical and rx costs. we give the pt a list of potential meds w/ the estimated copays.

Hi Missy,

In my transplant center the social worker handles the co-pays and patient assistance programs for the most part. I go over the coverage prior to transplant and work with the social worker and patient post-transplant on a requested basis only. When patients are called for transplant we ask them to bring or have someone bring their co-pays prior to discharge. Hope this helps!

We go over med coverage in education class with the patient.

Hello,

The TFC's verify rx coverage and go over it with the patients as well

as notify them of their out of pocket expense for drugs.  We also give

them the list of potential immunos they might be on as well as the

copays for those drugs.

Morning

The TFC does this at our center, when meeting with pt coverage is explained for medications also. We have list of most common medications used post transplant that we can give patients to have accurate estimate

on this benefit.

For those that obtain med benefit coverage, when explaining coverage to the

patient does that include their financial situation and if they are going to be

able to afford them? Just curious where you draw the line? I'm sure this has

been discussed before :)

Yes. we ask a lot of probing financial questions. Our goal is to be comfortable in the fact that the patient can afford the medication costs. Many times the patient will attempt to tell us what we want to hear. We generally probe them with questions until we can be pretty certain the information they are sharing with us is accurate.

Missy,

I do it here. I obtain copay amounts for

immunos, Valcyte,  Bactrim DS, Nystatin and Valtrex (just in case). I also go

over each and every cost – copays, surgical fees and professional fees

and estimated TOTAL monthly out of pocket costs for each patient.
