Good morning! Here’s my situation:

Patient has Medicare A,B,and D primary, BCBSKC Medigap secondary, and Kansas Medicaid tertiary. He is getting his premium for Medicare supplement paid for by NKF (which will end once transplanted). He has a large spend down with KS Medicaid so he will

probably not be able to us. He has very limited income (like $800/mth) Basically, once he is transplanted he will only have Medicare and the expense of 20% of meds, donut hole, and physician charges. What would you do in this situation? Would you approve

him for transplant?

Your patient has the option to continue his BCBSKC by paying the premiums

himself. This is where I encourage fund raising.

I would approve him if he will do the fund raising to a 3 month medication

amount, with the understanding this is an ongoing issue until he gets commercial

insurance.
