I asked Dr. Kolb the questions I presented to Dr. Melmed. As much as I worry about having few surgeons who will remove implants.... I have to say that if I had saline implants I would want the capsules removed, unless it was absolutely impossible (eg I would die if they were removed). Without better information from Dr. Melmed as to why he thinks saline capsules pose no problem, I don't think I would go to him. Sigh. I do trust Dr. Kolb on these issues. This is what she said: " You must remove the capsules in order to remove the biotoxin if the patient is to get well. You also need to treat with antifungals. " " Saline does cause platinun toxicity in some saline patients and it is important to get the capsules at for this reason as well as to get the biotoxin out of the patient. All saline disease I have seen is biotoxin related from mold in the implant associated with leaky valves. Pathology will never grow this out according to the lab director at Quest as this is normal room mold and not considered a pathogen. Dr. Blais sees this on microscopy as well as the leaky valves. " Somehow we have to get surgeons to investigate explant & explant complications in more depth. We can't have just one or two surgeons willing to do en bloc explants!!
