I dont think my surgeon does that and I dont want to take any chances............ Guest guest Posted August 30, 2005 Report Share Posted August 30, 2005 Diane, We thought Dr. Melmed, Dallas, TX was removing implants en bloc . . . but we found out that he was leaving part, if not the entire capsule. It's very upsetting because he has been our most outspoken advocate in the plastic surgery industry. A number of his patients have been having problems requiring aspiration (withdrawal of fluids) following surgery. Most doctors leave the drains in until there is no significant drainage. However, Dr. Melmed said that he removes the drains after one day so the women can leave for home. Even patients who have had problems think he is a wonderful doctor. He does beautiful work . . . My double lumen implants were ruptured, but Dr. Kolb said the saline was 'long gone'. Silicone had escaped the capsules, and Dr. Kolb got as much as she could, and took out the capsules. I am so grateful for that. I know thin capsules are not easy to get out, but I'm glad she did. I tend to agree, Lynda, that some PS might just not want to go to the effort to remove the capsules. That is why I would never go to any surgeon who is hostile to the idea that implants can cause problems. What would be their incentive for being meticulous about removing as much of that as possible? I am surprised about Dr. Melmed. I wonder if there is ever a legitimate reason a surgeon cannot remove the capsule? In all my years of implant support (7 and a half so far), I have only heard of ONE woman who experienced excessive bleeding and had to be taken to the emergency room at a hospital (her explant was being done in a clinic). She recovered fine and in fact, graduated with a PhD from an Eastern university not long after her experience. When Daryl and I were shopping for a plastic surgeon for her explant, we went to the head of the plastic surgery department at UNLV. We explained that we wanted a total capsulectomy and drains, no compromise on those two points. Boy, did he buck! He clearly pointed out to us that he never used drains and that as far as the total capsulectomy goes, he wouldn't guarantee that he could get everything out. He explained that sometimes the capsule is stuck to an area that is too hard to get to or the risk of bleeding makes it too dangerous to remove. He seemed to be making sure he had an open door to only remove what he wanted to. I certainly didn't feel that he had Daryl's best health interests in mind, and he wanted to make sure we knew who ran the show. He was an arrogant dude. This is the general attitude we find when trying to get a total capsulectomy or en bloc done. There is always some excuse for not being able to remove the whole thing. I think it is an issue of time, which equals money. A proper explant takes more time and skill to remove the whole capsule. Only the most ethical, compassionate and dedicated plastic surgeons are going to make sure they do it right, and those are few and far between. It's in the character of the person. Unfortunately, this specific medical field seems to be filled with car salesman " doctors " . It's a black mark on the whole field's reputation, in my opinion. I can believe there might be some instances where it would be more harmful than helpful to remove capsules with saline implants. I spoke with another woman who said Dr. Feng told her the same thing. One thing I wondered and have asked Dr. Melmed after I got this response (below) is what can a doctor do if the saline implant is contaminated (with fungus etc) and that contamination 'leaches' into the capsule... If the capsule happens to be one that is impossible to remove, what can be done? This is his response to my first question. I have just sent him my second question, so haven't yet got a response: " Gel capsules MUST be removed. The silicone leaches into the capsule wall. With SALINE implants, there is NO silicone in the capsules. Secondly, these capsules mostly dissolve within a year and pose no risk. Lastly sometimes they are impossible to remove without great risk, a risk far exceeding any theoretical benefit. I ALWAYS remove capsules in saline and gel IF POSSIBLE. There are some instances where it is impossible. And I can guarantee that there is not a surgeon alive who can remove these capsules without an enormous amount of damage: it is so flimsy, and astuck to ribs and muscle that in trying to remove these capsules, there is bleeding, tearing of the muscles, and when it is the muscles on the ribs, risk of perforation into the chest. To achieve what? In these insances, no-one can remove the capsule and surgeons who say they do simply are not telling the truth. " I am concerned about not recommending a surgeon who has been so involved and helpful to women on these issues. There are so few already who are willing to help us! I told him some women are very concerned. I also pointed out that we have good reason to be becasue so many women have been deceived by doctors on the issue of implants. I also am checking with other surgeons. Let's see if we can find an answer to these questions: 1. Is it true that capsules from saline implants are less dangerous than those from silicone implants? 2. Is it possible that contaminated saline can leach into the capsules and create a danger if not removed? 3. Are there times when it is impossible to remove capsules? 4. Is it possible for a surgeon to determine if the capsules from saline implants are contaminated. As far as I know there is none, and quite the contrary, they continue to evolve and morph into more dangerous masses. Dr. Pierre Blais has written a very detailed article on this subject, which also points out that capsules, whether from silicone gel or saline, have silicone particles embedded in them. I don't see Dr. Melmed offering any proof that capsules dissolve, and I certainly don't understand why he can claim that saline implants don't have silicone in the capsules. All saline implants have a silicone shell, and textured ones can slough off-- that means silicone particles--into the surrounding tissue--that means the scar capsule. I don't understand where these ideas arise from, other than somebody said it, so everyone believes it. I am not a doctor, but it seems to me that the idea that a capsule dissolves over time has been refuted. All silicon measurements were carried out using atomic absorption spectrometry with a graphite furnace. Silicon was measured in a normal heptane extract of silicone from dried tissue. The mean silicon levels in 16 breast tissue control samples from 8 patients undergoing breast reduction varied from 0.025 to 0.742 micrograms/gm with the median mean being 0.0927. The median silicon level in capsules from six patients with saline implants was 7.7 micrograms/gm (range; 1.9-36.6 micrograms/gm; averages 8,300% or 83 times more than those without a silicone implant). The median silicon level in capsules from five patients with silicone inflatable penile prostheses was 19.5 micrograms/gm (range; 1.9-34.8 micrograms/gm; .0927 divided by 19.5 = 21,000% more than those without any type silicone implant). Although the levels of silicon in capsules of patients with saline breast prostheses and penile implants were higher than in control samples, they were much lower than those from the capsules of the 58 gel implants (median; 9,979 micrograms/gm; range, 371-152,000 micrograms/gm; almost 108,000 times as much silicone in capsules Vs those without any type silicone implant). Another: Breast implant capsules are a foreign body immune response to breast implants. It has been proposed that capsulectomy after breast implant removal was unnecessary, as the body resorbs the capsule when the implant, the impetus for the foreign body response, is removed. We report eight women with persistent capsules 10 months to 17 years after silicone breast implant removal. NOPE--capsules don't dissolve! I noticed some changes almost immediately after explant (particularly the rash). It took over a year before I started feeling a lot better. Looking back, I realize I was improving, it just was gradual. I still have a ways to go. I had silicone implants too - actually 'double lumen' (silicone & saline) but the saline was long gone. The silicone implants were ruptured. I sure understand how you feel. I just posted a long message to Lea, explaining what a mess I was just a year and a half ago. It has been a short time, it just feels like forever. I have improved remarkably in that time. It makes such a difference. Do you know if your implant(s) are ruptured? I got one sports bra before explant, and a couple later - after I began healing. For awihle, I kept the velco 'wrap' that the surgeon used. Will you have drains? Guest guest Posted September 5, 2005 Report Share Posted September 5, 2005 Hi Melinda, If I remember correctly, Dr. Melmed is fully aware of the need for an en bloc removal for those with silicone gel implants. Perhaps since you have the gels, he will be sure to perform your explant correctly, and you can have peace about his skill in performing this surgery for you. I believe that Dr. Melmed has a good heart, and that he has a good sense of the dangers associated with implants. But perhaps you may want to get the details of your proposed surgery in writing from him, at least so you both know what is to be expected. 2 days with drains is better than no days with drains! I didn't have drains at all, and I am sorry that I didn't. I think it delayed my healing. Best wishes on your surgery. I know it seems so far off right now. He genuinely feels for women who have been through what we have. He told me that he removes capsules whenever he can, particularly with silicone implants. He said capsules surrounding silicone implants must be removed, because silicone leaches into the capsules. Dr. Melmed has asked me if there is somewhere he can post to give a more detailed explanation. I have suggested he post at a forum I moderate, and another forum where I better know the forum administrator. Would you all be willing to have him post? I didn't suggest this forum, because I don't know you all very well.
