We've had a rash of reports recently though. He is so charming, and his work is beautiful! . . . If only he could be counted on to remove saline implants en bloc, and use drains until there is no significant drainage. If I said shell, I misspoke . . . I meant the capsule (often called a scar tissue capsule). It has a blood supply though so, as I understand it, it's not really scar tissue. I've sent him information repeatedly about the necessity of removing the capsule . . . He told me he always removes them . . . but that's not what women are finding out after they start having problems. Apparently, he does remove all the capsules if the implant is silicone gel. Regardless, be sure to get a written committment from any doctor - prior to surgery that he/she will remove all the capsule. . . Or it may be necessary to remove the capsules later. I have gotten many recommendations for him from women who say he uses the en bloc technique. He has also told me he uses that technique. On another site, I read a post from a woman who was upset when she found out that he did not remove the entire capsule, and he responsed that he removed as much as he could safely. From what I understand, it isn't always possible to get out the entire capsule without damaging surrounding tissues--sometimes the capsules adhere to the rib cage, etc. I also know that some women go home with the drains Dr. Melmed puts in and have their own doctors take them out. I'm not defending Dr. Melmed, and I haven't seen him yet. I just wanted to relay what I have read about him. I am curious how many women have had unfortunate experiences with him. I find many doctors make excuses for not removing all of the capsule when a truly good plastic surgeon who believes they cause us problems will always do everything to get all of it. I recommend Dr. Feng and Dr. Huang usually, and I understand Dr. Kolb does remove the scar capsule. Of the three, Dr. Huang is about $5000 because of Denver location, and the others are twice that. Insurance sometimes covers this surgery. Some are fortunate that way. This means that a large portion of the silicone shell was in my body and hopefully caught in the scar capsule.... For a doctor to not remove the entire capsule (especially one who says he believes in implant illness) is insane if you ask me.... the shell breaks down - that has been proven over and over again. As it does this the silicone in the shell goes elsewhere - the obvious place for a lot of it to be would be hung up in the scar capsule if it is intact. This is why I think that is sooooo important to have a doctor that removes the entire capsule - no matter how thick or thin it is. So what if it takes a little rib scraping? Isn't that what we are paying these surgeons top dollar for? I had a pretty good scraping done to mine to get the capsules out and yeah - it wasn't too pleasant and I had extra rib pain and issues but I was happy to know that it was out! What ever silicone escapes my capsules over the years is lost - I have to work on getting that out myself. At least we need to know that our surgeons are getting every last bit of implant related tissues outta there! Scar capsule and all - think of all the massaging they made you do before you realized implants were bad.... I'll bet every time we did it a little particle broke off into the scar capsule. I just cannot believe that there are docs out there that claim to understand implant illness and do not take out the entire capsules. Who would want that nasty scar tissue floating around in their chest anyways? I was so disgusted with my implants by the time I had them removed I didn't care if they had to take half my ribs with them as long as all of it was out of me!!!! I didn't care about the after pain or how big the scar was and I told my doctor so.... just get rid of them and everything else w/ them!! He did a great job and I ended up with very minimal scarring - I still have some rib pain as he had to do a fair amount of scraping but I am happy with that. At least I know they are all gone.... the rest will have to work it's way out through my detoxing. I would not go to a doctor if I were awaiting explant who did not say up front that no matter what he removes all of the capsules/scar tissue. No excuses on why not to..... I know mine were under the muscle for 7 years and one adhered pretty good to my chest wall. I am sure there are good reasons doctors can give for everything - I just can not be sure that they are honest as I have a lot of trouble trusting anyone in the medical (plastic surgery) field these days. I was told a lot of things from my original PS - leaks are very uncommon (had a complete leak after 3 years), capsular contracture was very rare (had to have surgery as my CC got so bad after only 7 months after implantation) he said if I removed them I would look horrible and deformed.... they have been out for 2 years and I didn't even have a lift and they look completely normal.... I could never attempt to answer a surgical question like that as I am sure there are cases where bleeding may be an issue or the capsule is near the heart or lungs... who is to say? I am sure those things and many more can happen and I could never really understand any of them as I have never had any type of medical training. I just know that people get sick from implants and a lot of the illnesses are caused by exposure to the silicone shell - that much I can say so I would be reluctant to get them removed without worrying about some of that being left behind and what that may cause medically after time goes on.... maybe it is harmlessly absorbed ... maybe your body can detox the capsule and silicone and you could completely be healed afterwards..... everyone is different so you really never can tell. I guess were I in that situation again, (needing explant) I would just have to look and interview as many doctors as it would take to be completely trusting of them and my health. there probably isn't really a single answer for any question as I am sure all womens implant situations are different ... they are probably all different once a doctor opens you up to see what is going on - maybe they can't really effectively answer the question before they get in there and see what is going on... shari Quote Link to comment Share on other sites More sharing options...

They feel very betrayed . . . especially after having problems and traveling so far. One woman traveled from Mexico City. One woman was told the capsule was attached to the ribs. . . with overs???? That doesn't make sense. Granted, there may be times when a doctor can't remove everything. That's when women need to have a doctor they can trust. If fact, I thought Orthopedic Surgeons were arrogant until I met Plastic Surgeons. My experience has been that the majority are unwilling to be told how you want surgery done. That is why I only recommend a few that I know are not like that and will do it right. I am very skeptical of most who say they " will try to get it all out " . I just do not trust them. I was so fortunate . . . I really think it was God that connected me to my surgeon and held his hand. I was terribly ill at the time . . . He probably didn't remove them en bloc, but he did do a complete capsulectomy. . . and told me, after the surgery, that he had cleaned me out throughly. To this day, he usually leaves the capsules. He got really angry when I brought up the subject of implants and illness . . . I guess they have to find a way of justifying what they do! I tried to connect him with Dr. Blais since I met him while accompanying a woman to see him for her initial consult about six years ago. I'm very disappointed in his position. I thought he was one of our best doctors until I started getting reports of capsules left behind. He told me the pathologist he uses had never found mold/fungus in saline implants. . . .????? Maybe I should mail him my capsules..... full of mold and pieces of textured silicone shell.... I know they only made the textured implants for a short period but thousands and thousands got them and this is most like what is happening to all of them... as Dr. Blais' said - the textured shells shed.... particles get into the scar capsule and beyond... it actually makes very good sense and I wouldn't have to even be a doctor to understand that logic. Take a ribbed (textured) implant. Stick it inside a 98 degree body where a scar capsule forms around it to hold it in place.... Massage the crap out of it constantly to avoid capsular contracture (for the second time in my case) by breaking up the scar tissue around it so they stay soft.... Massage them daily as instructed by your PS..... leave them in for 7 years and who thinks that "ribbed" silicone on the outside of those implants wouldn't be breaking off along with the scar tissue you are constantly massaging off? Upon removal one of my implants was smooth as it had to be replaced in 99 due to a complete rupture.... they did not choose to use a textured one again for whatever reason.... My right implant was the original... 30% of the shell was missing.... there is actually smooth spots on it visible through all the mold. Where does the doctor think this "ribbing" had gone? It isn't on the implant anymore - I can prove that.... did it disappear into thin air? no - it was inside of my capsules and beyond.... large pieces of silicone throughout my body - this is why Dr. Vasey thinks I got so sick - I have systematic silicon disease" as he puts it - all the same symptoms are there as the gel patients he sees. I find it hard to believe that this or any doctor would say that there can be no silicone in the capsules when a patient is implanted w/ silicone shells not matter what the filling.
