Wow! I was just reading the first few pages of this guy's book and it is so clear and simple & yet at the same time so hard, so tough to find 500 to 750, perhaps as many as a 1,000 patients who so get it, and have the means to pay for it so as to return to this so needed, lost and wonderful possibility that in one way or another we here are all attempting to reach. Some more successfully than others. He just lays it out so nicely and cleanly. It so simple, so obvious...

I just had some rude women call the office and she was leaving a message about how she was going to note and not cooperating with the quality review requesting charts and the like, typical managed care, FHP's and CHIP's BS. And so I picked her up in mid-sentence and almost took her head off I was so pissed off while listening to her while at the same time almost in tears as I was getting things ready for the main weekly bank run. She was so suprised to be treated so agressively by me, and I said, "yeah, because I'm not just another leech living off some over worked under paid doctor, I'm the freakin' doctor's husband and me and my kids have to live with the results of companies like yours. I gave her gruff about all the uncompensated BS paperwork and why should some bozo with a BA hold power of authorization over the choices made by my wife who has three times the training. I asked just what do you think she studied and learned in Medical

School???? ANd then I answered my own question with: "She learned how to managed patients' care, how to care for sick people and take good care of them and their diseases that's what..." And then I asked who should be the actual judge of the quality of the doctor's work? Like shouldn't it be the patient's choice as whether or not the doctor is providing them with good care or not? Are they not the actual consumer of my wive's services not your company?

She went on about how I was the first person to challenge her on such things and I should have told Yeah but I won't be the last that's for sure but it didn't come to me then on the phone. I told her that copying records for their useless reviews is low priority around her when almost every patient visit has some sort of studid and needless paperwork to be done for referals and prior auth's fo services, test and Rx's. I told her we had to can our MA because every year companies like hers demand more and more paperwork "managing care" like they know what the heck they are doing, and yet they never pay more for all the garbage they demand... I told her she'll get her records "eventually" after my wife gets done dealing with the 10 other phonecalls and other BS prior auths she needs to do for stupid companies like hers.

Then she told me that this was her job and that really sent me off. I told her straight up well you see you have a bad job where you are part of doing things that are a waste of time, add needless expense and wasted time to the process where you see I have a good job because I am actually attempting to assist the doctor here in actually providing care for the patients, like the one's in her plan that don't pay nearly enough for all that they demand out of my wife, instead of draining the system for my own exsistance on the grounds of some false claim of checking up on the quality of care provided by the doctor's forced to do business with a plan they would never willing sign on with if they felt they could do otherwise.... Oh and she kept going on about how she attempted to reach us five times and had we simply replied as some point earlier on.... As though on some other day things would have been much different from the mounds of

crap that flows thru here everyday parading around as some form of care or concern for quality or managment when clearly it is only to limit access and limit payment for services and care.

She then said well she could have come by and copied the records herself had we only spoken with her and I quickly responded back, like hell you ever will. I have the privacy of a thousand patients to protect and look after and I will never allow an outsider to simply sit at our computers and rummage thru them to their heart's content. AND.... Now I just spoke with who says she actually sent this women a response and informed her that we are in the midst of a computer and software upgrade and we literally can't access what she is requesting... But that we are not ignoring you and that we can and will send what was requested once all the upgrades and related glitches are taken care of... As soon as we can get inside the system. And this is all true. We just updated to AC 5.0 which seems to be cooperating fairly well actually and other than one interesting hang up with the peer computers even the update from fairly old 3.7 to

newest 5.0 went pretty darn well. But originally we were updating FAP, file assist pro our side software for better document managment than in AC. It is FAP that is hanging up and we simply can not access all our scanned in stuff right now until I get the two talking smoothly again between one another. She wants labs and I can not get to printed and scanned labs. Only the Quest labs can be accessed because those are "E" and go direct into AC very nicely. But if the patients use any of the other labs those are paper or fax and they are stuck in dysfunctional FAP for the moment, what can I say? So here is this big "B" and I do mean with a capital B mind you threatening us even though she was given a legit reason as to why we can't give her what she wants. And the interesting thing here really is, that the carrier paid for the labs and that the lab is another "private contractor" that they have a contract with, and so why doesn't she simply go get her labs

from the other provider who actually ran the tests in the first place... They too should have the records to share and keep safe for years to come and there is the answer and satisfaction of her request which I was not aware of at the time of the call I fielded... What a moron, what a PITA.

Boy I flamed the hell out of this women. And I think had her tone been more agreeable and less I'm the big "B" who is going to threaten you like some full of themselves middle school teacher, I would not have jumped down her throat so bad... But it was almost like "This is going on your permenent record Mr !" kind of control "B" attitude as though it has come to this that doctors have to kiss the butt and be scared of some idiot, middle nobody threatening them like this because their software is having some delays and difficulties... As though their studip little pay poorly for FHP and CHIP plan shouldn't be estatic to have a solo doc who actually see her patients for 20-40 minutes at a clip and uses a (GASP!!!!) CCHIT certified EMR no less... GIve me an freakin' break.... I was shaking by the time I got off the phone with this women, really I was... I just was not going to be intimidated and pushed around by someone who

shouldn't really even have a job in the first place. As they said in the Bergus Merideth Twilight Zone when he was a Librarian... "You are Obsolete". These care managment idiots, they are Obsolete....

We blinked and an entire industry that has nothing to do with care, grew up, around, under, over and through healthcare. Most of the people in this industry of minutiae really think they are doing well by doing good. They have no idea of the complexity and nuance of human beings and health. They don't see the people for all of the paper. And of course this population also has its proportionate share of jerks like the one you encountered today.If I had it in my power, I would send you the gift of the largest, most beautifully smooth ice in the world so you could lace up your skates and tear it up. All I can say is that you have put a smile on my face a couple of times over the last day or two with Rocket J and Bullwinkle, and now this supportive, empathic response. It more valueable than you might every know. Thanks, really. After being here at the office two nights in a row, to get home so late as to say it was "early" instead of late, I have just about had my fill of all the BS that is this business. First it was like 3:30 am before I got done eating my "dinner", fed the cats, took 5 mgs of Ambien to take the edge off being wired but tired and hit the hay Tuesday night. Oh and the 200K mile old '98 Jeep finally decided it didn't want to hold an idle when I went out into the parking lot to go home too... So back into the office to get some tools, (1/2 " open end wrench basically) to disconnect the battery for a moment in hope that rebooting the things computer would clear it's brain and or help the IAC (Idle Air

Controller) zero itself out, run a self diagnostic and return to some amount of a correct position so as to be able to drive home... At first it seemed I was out of luck, but then I kicked in the A/C switch which sends signals to the IAC as well to compensate for load differences with compressor on or off cycle and that seemed to do the trick... (Jack of all trades master of goodness knows what....) And then last night I only stayed til about 1am and caught the re-broadcast of surfing back and forth between Countdown for a political fix and In Plain Sight for my silly cop drama fix (Law and Order's , CSI, CSI NY, Burn Notice). Needless to say I have not been getting in too early that last few days either...

But man did this women push the wrong man's buttons on the wrong day. I have just about had as much as I can take, especially because I have no idea what I am going to do about paying for our own family's health insurance or 's over Due Med Mal (AGAIN!!!). It is just insane...

We don't get paid enough from the insurance industry so we can continue to pay all the various parts both gov't as well as private, of that same insurance industry to keep our doors open and our butts properly covered... It really is just about totally insane when you really stop and look at it in it's proper context. Our entire economic and professional lives are 100% controlled in a negative cash flow, downward spiral, by the same industry just on opposite sides of the same coin, really.... One side over demands and under compensates and then has almost all the laws and rules written in such a way that we don't dare not to purchase all sorts of over priced policies from them to cover our backsides.. Some we are not even allowed to do without even if we wanted to go naked. Right now we have an outstanding major penalty to fight and probably lose, from NYS Worker's Comp because we had a lasp in coverage for awhile the last time I

had to find some money to pay med mal last quarter....

As I posted a ways back, we here in primary care are taking it from every side in a form that is truly a negative cash flow model. I really feel like one of those pre-union coal miners in "16 Tons", being forced to rent a shack from the company at some rediculously inflated price while we buy our groceries and other necessities at the company store, and every day getting another day older and deeper in debt... "St don't ya call me 'cause I can't go, I owe my soul to the company store..."

Anyway the Moose is not copied and in a folder on my laptop.... :) Oh, and I have been told by my family that at about 3 years old I was an avid Crusader Rabbit fan too (same cast of R & B nut jobs)... Thanks again.

Hey . I haven't been on this site in awhile. Saw this post.

A couple of days ago, I was (still am) feeling just washed out on this whole type of business. I looked a Alice and said "This is a dog business. Primary Care Medicine, in general, when paid for by insurance is a dog business. She wasnt' happy. BUT IT IS. And you dont need an MBA (which i happen to have) to konw it.

I just had some rude women call the office and she was leaving a message about how she was going to note and not cooperating with the quality review requesting charts and the like, typical managed care, FHP's and CHIP's BS. And so I picked her up in mid-sentence and almost took her head off I was so pissed off while listening to her while at the same time almost in tears as I was getting things ready for the main weekly bank run. She was so suprised to be treated so agressively by me, and I said, "yeah, because I'm not just another leech living off some over worked under paid doctor, I'm the freakin' doctor's husband and me and my kids have to live with the results of companies like yours. I gave her gruff about all the uncompensated BS paperwork and why should some bozo with a BA hold power of authorization over the choices made by my wife who has three times the training. I asked just what do you think she studied and learned in Medical

School???? ANd then I answered my own question with: "She learned how to managed patients' care, how to care for sick people and take good care of them and their diseases that's what..." And then I asked who should be the actual judge of the quality of the doctor's work? Like shouldn't it be the patient's choice as whether or not the doctor is providing them with good care or not? Are they not the actual consumer of my wive's services not your company?

She went on about how I was the first person to challenge her on such things and I should have told Yeah but I won't be the last that's for sure but it didn't come to me then on the phone. I told her that copying records for their useless reviews is low priority around her when almost every patient visit has some sort of studid and needless paperwork to be done for referals and prior auth's fo services, test and Rx's. I told her we had to can our MA because every year companies like hers demand more and more paperwork "managing care" like they know what the heck they are doing, and yet they never pay more for all the garbage they demand... I told her she'll get her records "eventually" after my wife gets done dealing with the 10 other phonecalls and other BS prior auths she needs to do for stupid companies like hers.

Then she told me that this was her job and that really sent me off. I told her straight up well you see you have a bad job where you are part of doing things that are a waste of time, add needless expense and wasted time to the process where you see I have a good job because I am actually attempting to assist the doctor here in actually providing care for the patients, like the one's in her plan that don't pay nearly enough for all that they demand out of my wife, instead of draining the system for my own exsistance on the grounds of some false claim of checking up on the quality of care provided by the doctor's forced to do business with a plan they would never willing sign on with if they felt they could do otherwise... . Oh and she kept going on about how she attempted to reach us five times and had we simply replied as some point earlier on.... As though on some other day things would have been much different from the mounds

of crap that flows thru here everyday parading around as some form of care or concern for quality or managment when clearly it is only to limit access and limit payment for services and care.

She then said well she could have come by and copied the records herself had we only spoken with her and I quickly responded back, like hell you ever will. I have the privacy of a thousand patients to protect and look after and I will never allow an outsider to simply sit at our computers and rummage thru them to their heart's content. AND.... Now I just spoke with who says she actually sent this women a response and informed her that we are in the midst of a computer and software upgrade and we literally can't access what she is requesting.. . But that we are not ignoring you and that we can and will send what was requested once all the upgrades and related glitches are taken care of... As soon as we can get inside the system. And this is all true. We just updated to AC 5.0 which seems to be cooperating fairly well actually and other than one interesting hang up with the peer computers even the update from fairly old 3.7 to

newest 5.0 went pretty darn well. But originally we were updating FAP, file assist pro our side software for better document managment than in AC. It is FAP that is hanging up and we simply can not access all our scanned in stuff right now until I get the two talking smoothly again between one another. She wants labs and I can not get to printed and scanned labs. Only the Quest labs can be accessed because those are "E" and go direct into AC very nicely. But if the patients use any of the other labs those are paper or fax and they are stuck in dysfunctional FAP for the moment, what can I say? So here is this big "B" and I do mean with a capital B mind you threatening us even though she was given a legit reason as to why we can't give her what she wants. And the interesting thing here really is, that the carrier paid for the labs and that the lab is another "private contractor" that they have a contract with, and so why doesn't she simply go get her labs

from the other provider who actually ran the tests in the first place... They too should have the records to share and keep safe for years to come and there is the answer and satisfaction of her request which I was not aware of at the time of the call I fielded... What a moron, what a PITA.

Boy I flamed the hell out of this women. And I think had her tone been more agreeable and less I'm the big "B" who is going to threaten you like some full of themselves middle school teacher, I would not have jumped down her throat so bad... But it was almost like "This is going on your permenent record Mr !" kind of control "B" attitude as though it has come to this that doctors have to kiss the butt and be scared of some idiot, middle nobody threatening them like this because their software is having some delays and difficulties. .. As though their studip little pay poorly for FHP and CHIP plan shouldn't be estatic to have a solo doc who actually see her patients for 20-40 minutes at a clip and uses a (GASP!!!!) We blinked and an entire industry that has nothing to do with care, grew up, around, under, over and through healthcare. Most of the people in this industry of minutiae really think they are doing well by doing good. They have no idea of the complexity and nuance of human beings and health. They don't see the people for all of the paper. And of course this population also has its proportionate share of jerks like the one you encountered today.

If I had it in my power, I would send you the gift of the largest, most beautifully smooth ice in the world so you could lace up your skates and tear it up. Then I would have a fairy godmother hand deliver cocoa for the kids, beers for you and and pizza all around.

Yeah I sure did, didn't I??? In the words of Bugs Bunny: "Ain't I a stinker?" Now there's a real NY accent for ya. But yes I was just about to the point of blowing and in the dumps as I was looking at our pathetic returns and what little I had to deposit to cover many more bills then we have money.

But I think it is a real riot that folks like us are so caught between the two sides of the same coin that really is bleeding most of America dry, private for profit insurance. And it is so fun, so upsetting that most folks just don't get it yet or at least those that do get it, are not given any major press time while the relatively low mass until given press time, Astro Turf organizations like the Tea Party and other conservative false movements do so they are growing. These bastards really are not slowly snuffing all of us out and I don't mean just docs, I mean almost all Americans and most American businesses too. The recent posts about the research showing that as much as 40% of all doctor profits can just be taken right off the top in service to our corporate masters, the Mega Insurance companies...

So on one side we don't get paid enough to survive on, no less actually make a reasonable living. And yet on the opposite side of the same exact coin, these uncontrolled SOB's are over charging us for every one of their products that we are required or better have if we know what is good for us, be it business insurance, WC's, Disability, Health Insurance of our Own, Med Mal and all the rest. When combined with our other wonderful friends (who certainly have our best interests at heart) in the banking industry who lend us back our own money at multiple times more than we lent it to them in the first place, it's a wonder any small or moderate sized business is left standing in the country what so ever, no less those of us who are stuck actually making our daily bread off of the meger scraps we are fed from the Insurance industry.

I still say: "Beware of the Medical Industrial Complex!" Almost everyone else in this game is not price controlled and has a much more liberal and real free market to hit us all up with, but some how we are the only ones who are really tied down with the price controls artificially set and in place by the gov't wonderful handling of the Medicare fee schedule and the stupid, anti-free trade billing and coding system. Big Pharma isn't price controlled, neither are the software, medical suppliers and supply houses (except DME that is), GE, WA, Next Gen, BC/BS, UHC, and all the rest, but we are... How much did your health insurance premiums go up this year and I'm sure that your fees from those same carriers certainly didn't go up proportionally if at all... Right?

Anyway, I have missed see you too and as you can tell I am still attempting to fight the current and not get washed away thru the rapids and over the falls.... I'm going to be working on the phone and on-line with Bert tonight at around 7 pm to hopefully get this FAP thing all straightened out... Gosh, , that really made you mad, didn't it?What I do when I get mad about these reviews is call the patient and ask if they told their insurance company it was all right to have a stranger from another company associated with them (they are all outsourced) read their personal health records including any counseling I might have done. They all say "NO!" Then I give them the phone number, and the addresses of anyone important...you see, patients have the right to deny access to these specific office notes. They just don't have the right to defraud the insurance company by are hiding some diagnosis they don't want them to know. So.... I get the patient to call and harass them. So far it has worked like a charm. I get these letters rescinding their requests for records. Nice, don't

We just updated to AC 5.0 which seems to be cooperating fairly well actually and other than one interesting hang up with the peer computers even the update from fairly old 3.7 to

newest 5.0 went pretty darn well. But originally we were updating FAP, file assist pro our side software for better document managment than in AC. It is FAP that is hanging up and we simply can not access all our scanned in stuff right now until I get the two talking smoothly again between one another. She wants labs and I can not get to printed and scanned labs. Only the Quest labs can be accessed because those are "E" and go direct into AC very nicely. But if the patients use any of the other labs those are paper or fax and they are stuck in dysfunctional FAP for the moment, what can I say? So here is this big "B" and I do mean with a capital B mind you threatening us even though she was given a legit reason as to why we can't give her what she wants. And the interesting thing here really is, that the carrier paid for the labs and that the lab is another "private contractor" that they have a contract with, and so why doesn't she simply go get her labs

from the other provider who actually ran the tests in the first place... They too should have the records to share and keep safe for years to come and there is the answer and satisfaction of her request which I was not aware of at the time of the call I fielded... What a moron, what a PITA.

Boy I flamed the hell out of this women. And I think had her tone been more agreeable and less I'm the big "B" who is going to threaten you like some full of themselves middle school teacher, I would not have jumped down her throat so bad... But it was almost like "This is going on your permenent record Mr !" kind of control "B" attitude as though it has come to this that doctors have to kiss the butt and be scared of some idiot, middle nobody threatening them like this because their software is having some delays and difficulties. .. As though their studip little pay poorly for FHP and CHIP plan shouldn't be estatic to have a solo doc who actually see her patients for 20-40 minutes at a clip and uses a (GASP!!!!) CCHIT certified EMR no less... GIve me an freakin' break.... I was shaking by the time I got off the phone with this women, really I was... I just was not going to be intimidated and pushed around by someone who

Wow. I'm totally shot after being here like four nights in a row past midnight working of the #$% & #* computers... How come you have never shared this awesome gem with us before this? Almost all of our patients are super pro privacy and they get that. Why I even have a few "attack dog" like ones who like myself would get off on it from time to time. I love sicking the irrate patients of carriers any chance I get. It saves us the trouble and it makes the carriers side all the more messed up and makes them back down infront of the customer who they don't want to piss off quite as quickly as us.... I can't wait to use this the very next chance I get... Now to back up AC and drag my sorry tush home ASAP....

Gosh, , that really made you mad, didn't it?What I do when I get mad about these reviews is call the patient and ask if they told their insurance company it was all right to have a stranger from another company associated with them (they are all outsourced) read their personal health records including any counseling I might have done. They all say "NO!" Then I give them the phone number, and the addresses of anyone important... you see, patients have the right to deny access to these specific office notes. They just don't have the right to defraud the insurance company by are hiding some diagnosis they don't want them to know. So.... I get the patient to call and harass them. So far it has worked like a charm. I get these letters rescinding their requests for records. Nice, don't you think?

I just had some rude women call the office and she was leaving a message about how she was going to note and not cooperating with the quality review requesting charts and the like, typical managed care, FHP's and CHIP's BS. And so I picked her up in mid-sentence and almost took her head off I was so pissed off while listening to her while at the same time almost in tears as I was getting things ready for the main weekly bank run. She was so suprised to be treated so agressively by me, and I said, "yeah, because I'm not just another leech living off some over worked under paid doctor, I'm the freakin' doctor's husband and me and my kids have to live with the results of companies like yours. I gave her gruff about all the uncompensated BS paperwork and why should some bozo with a BA hold power of authorization over the choices made by my wife who has three times the training. I asked just what do you think she studied and learned in Medical

School???? ANd then I answered my own question with: "She learned how to managed patients' care, how to care for sick people and take good care of them and their diseases that's what..." And then I asked who should be the actual judge of the quality of the doctor's work? Like shouldn't it be the patient's choice as whether or not the doctor is providing them with good care or not? Are they not the actual consumer of my wive's services not your company?

She went on about how I was the first person to challenge her on such things and I should have told Yeah but I won't be the last that's for sure but it didn't come to me then on the phone. I told her that copying records for their useless reviews is low priority around her when almost every patient visit has some sort of studid and needless paperwork to be done for referals and prior auth's fo services, test and Rx's. I told her we had to can our MA because every year companies like hers demand more and more paperwork "managing care" like they know what the heck they are doing, and yet they never pay more for all the garbage they demand... I told her she'll get her records "eventually" after my wife gets done dealing with the 10 other phonecalls and other BS prior auths she needs to do for stupid companies like hers.

Then she told me that this was her job and that really sent me off. I told her straight up well you see you have a bad job where you are part of doing things that are a waste of time, add needless expense and wasted time to the process where you see I have a good job because I am actually attempting to assist the doctor here in actually providing care for the patients, like the one's in her plan that don't pay nearly enough for all that they demand out of my wife, instead of draining the system for my own exsistance on the grounds of some false claim of checking up on the quality of care provided by the doctor's forced to do business with a plan they would never willing sign on with if they felt they could do otherwise... . Oh and she kept going on about how she attempted to reach us five times and had we simply replied as some point earlier on.... As though on some other day things would have been much different from the mounds

of crap that flows thru here everyday parading around as some form of care or concern for quality or managment when clearly it is only to limit access and limit payment for services and care.

She then said well she could have come by and copied the records herself had we only spoken with her and I quickly responded back, like hell you ever will. I have the privacy of a thousand patients to protect and look after and I will never allow an outsider to simply sit at our computers and rummage thru them to their heart's content. AND.... Now I just spoke with who says she actually sent this women a response and informed her that we are in the midst of a computer and software upgrade and we literally can't access what she is requesting.. . But that we are not ignoring you and that we can and will send what was requested once all the upgrades and related glitches are taken care of... As soon as we can get inside the system. And this is all true. Joanne, I was SOOOOOOOOO HOPING my patients would do the same thing! Everyone of them said it was okay to send their whole chart to the insurance company! I made sure I mendtioned privacy and counseling, etc. They didn't "bat an eye!".

In fine print, think it permits ins co to do this.

I'm starting to have "discs" for all records requests -- cost is $1.22 to mail locally.

Also, remember that if sending, use your state medical records copy fees and BE SURE THEY PAY YOU FIRST per page EVEN IF ELECTRONIC.

I told her we had to can our MA because every year companies like hers demand more and more paperwork " managing care " like they know what the heck they are doing, and yet they never pay more for all the garbage they demand... I told her she'll get her records " eventually " after my wife gets done dealing with the 10 other phonecalls and other BS prior auths she needs to do for stupid companies like hers.

 

    Then she told me that this was her job and that really sent me off. I told her straight up well you see you have a bad job where you are part of doing things that are a waste of time, add needless expense and wasted time to the process where you see I have a good job because I am actually attempting to assist the doctor here in actually providing care for the patients, like the one's in her plan that don't pay nearly enough for all that they demand out of my wife, instead of draining the system for my own exsistance on the grounds of some false claim of checking up on the quality of care provided by the doctor's forced to do business with a plan they would never willing sign on with if they felt they could do otherwise... . Oh and she kept going on about how she attempted to reach us five times and had we simply replied as some point earlier on.... As though on some other day things would have been much different from the mounds of crap that flows thru here everyday parading around as some form of care or concern for quality or managment when clearly it is only to limit access and limit payment for services and care.

 

    She then said well she could have come by and copied the records herself had we only spoken with her and I quickly responded back, like hell you ever will. I have the privacy of a thousand patients to protect and look after and I will never allow an outsider to simply sit at our computers and rummage thru them to their heart's content. AND.... Now I just spoke with who says she actually sent this women a response and informed her that we are in the midst of a computer and software upgrade and we literally can't access what she is requesting.. . But that we are not ignoring you and that we can and will send what was requested once all the upgrades and related glitches are taken care of... As soon as we can get inside the system. And this is all true. We just updated to AC 5.0 which seems to be cooperating fairly well actually and other than one interesting hang up with the peer computers even the update from fairly old 3.7 to newest 5.0 went pretty darn well. But originally we were updating FAP, file assist pro our side software for better document managment than in AC. It is FAP that is hanging up and we simply can not access all our scanned in stuff right now until I get the two talking smoothly again between one another. She wants labs and I can not get to printed and scanned labs. Only the Quest labs can be accessed because those are " E " and go direct into AC very nicely. But if the patients use any of the other labs those are paper or fax and they are stuck in dysfunctional FAP for the moment, what can I say? So here is this big " B " and I do mean with a capital B mind you threatening us even though she was given a legit reason as to why we can't give her what she wants. And the interesting thing here really is, that the carrier paid for the labs and that the lab is another " private contractor " that they have a contract with, and so why doesn't she simply go get her labs from the other provider who actually ran the tests in the first place... They too should have the records to share and keep safe for years to come and there is the answer and satisfaction of her request which I was not aware of at the time of the call I fielded... What a moron, what a PITA.

And I think had her tone been more agreeable and less I'm the big " B " who is going to threaten you like some full of themselves middle school teacher, I would not have jumped down her throat so bad... But it was almost like " This is going on your permenent record Mr ! " kind of control " B " attitude as though it has come to this that doctors have to kiss the butt and be scared of some idiot, middle nobody threatening them like this because their software is having some delays and difficulties. .. As though their studip little pay poorly for FHP and CHIP plan shouldn't be estatic to have a solo doc who actually see her patients for 20-40 minutes at a clip and uses a (GASP!!!!) CCHIT certified EMR no less... GIve me an freakin' break.... I was shaking by the time I got off the phone with this women, really I was... I just was not going to be intimidated and pushed around by someone who shouldn't really even have a job in the first place. reading the first few pages of this guy's book and it is so clear and simple

& yet at the same time so hard, so tough to find 500 to 750, perhaps as

many as a 1,000 patients who so get it, and have the means to pay for it so

I just had some rude

women call the office and she was leaving a message about how she was going

to note and not cooperating with the quality review requesting charts

and the like, typical managed care, FHP's and CHIP's BS. And so I picked her

up in mid-sentence and almost took her head off I was so pissed off while

listening to her while at the same time almost in tears as I was getting

things ready for the main weekly bank run. She was so suprised to be treated

so agressively by me, and I said, " yeah, because I'm not just another

leech living off some over worked under paid doctor, I'm the freakin'

doctor's husband and me and my kids have to live with the results of

companies like yours. I gave her gruff about all the uncompensated BS

paperwork and why should some bozo with a BA hold power of authorization over

the choices made by my wife who has three times the training. I asked

just what do you think she studied and learned in Medical School????

ANd then I answered my own question with: " She learned how to managed

patients' care, how to care for sick people and take good care of them and

their diseases that's what... " And then I asked who should be the actual

judge of the quality of the doctor's work? Like shouldn't it be the patient's

choice as whether or not the doctor is providing them with good care or not?

Are they not the actual consumer of my wive's services not your company?

She

went on about how I was the first person to challenge her on such things and

I should have told Yeah but I won't be the last that's for sure but it didn't

come to me then on the phone. I told her that copying records for their

useless reviews is low priority around her when almost every patient visit

has some sort of studid and needless paperwork to be done for referals and

prior auth's fo services, test and Rx's. I told her we had to can our MA

of a computer and software upgrade and we literally can't access what she is

requesting.. . But that we are not ignoring you and that we can and will send

what was requested once all the upgrades and related glitches are taken care

of... As soon as we can get inside the system. And this is all true. We just

updated to AC 5.0 which seems to be cooperating fairly well actually and

other than one interesting hang up with the peer computers even the update

from fairly old 3.7 to newest 5.0 went pretty darn well. But originally we

were updating FAP, file assist pro our side software for better document

managment than in AC. It is FAP that is hanging up and we simply can not

access all our scanned in stuff right now until I get the two talking

smoothly again between one another. She wants labs and I can not get to

printed and scanned labs. Only the Quest labs can be accessed because those

are " E " and go direct into AC very nicely. But if the patients use

any of the other labs those are paper or fax and they are stuck in dysfunctional

FAP for the moment, what can I say? So here is this big " B " and I

do mean with a capital B mind you threatening us even though she was given a

legit reason as to why we can't give her what she wants. And the interesting

thing here really is, that the carrier paid for the labs and that the lab is

another " private contractor " that they have a contract with, and so

Will look for smaller envelopes, but I do put a plastic rigid holder around the CD.

Actually the pt I was going to send off the CD for is coming back to me as a pt..........

I'll be using this method in the future, however, so am moving ahead with this anyway.

I told her that copying records for their useless reviews is low priority around her when almost every patient visit has some sort of studid and needless paperwork to be done for referals and prior auth's fo services, test and Rx's. I told her we had to can our MA because every year companies like hers demand more and more paperwork "managing care" like they know what the heck they are doing, and yet they never pay more for all the garbage they demand... I told her she'll get her records "eventually" after my wife gets done dealing with the 10 other phonecalls and other BS prior auths she needs to do for stupid companies like hers.

Then she told me that this was her job and that really sent me off. I told her straight up well you see you have a bad job where you are part of doing things that are a waste of time, add needless expense and wasted time to the process where you see I have a good job because I am actually attempting to assist the doctor here in actually providing care for the patients, like the one's in her plan that don't pay nearly enough for all that they demand out of my wife, instead of draining the system for my own exsistance on the grounds of some false claim of checking up on the quality of care provided by the doctor's forced to do business with a plan they would never willing sign on with if they felt they could do otherwise... . Oh and she kept going on about how she attempted to reach us five times and had we simply replied as some point earlier on.... As though on some other day things would have been much different from the mounds of crap that flows thru here everyday parading around as some form of care or concern for quality or managment when clearly it is only to limit access and limit payment for services and care.

She then said well she could have come by and copied the records herself had we only spoken with her and I quickly responded back, like hell you ever will. I have the privacy of a thousand patients to protect and look after and I will never allow an outsider to simply sit at our computers and rummage thru them to their heart's content. AND.... Now I just spoke with who says she actually sent this women a response and informed her that we are in the midst of a computer and software upgrade and we literally can't access what she is requesting.. . But that we are not ignoring you and that we can and will send what was requested once all the upgrades and related glitches are taken care of... As soon as we can get inside the system. And this is all true. We just updated to AC 5.0 which seems to be cooperating fairly well actually and other than one interesting hang up with the peer computers even the update from fairly old 3.7 to newest 5.0 went pretty darn well. But originally we were updating FAP, file assist pro our side software for better document managment than in AC. It is FAP that is hanging up and we simply can not access all our scanned in stuff right now until I get the two talking smoothly again between one another. She wants labs and I can not get to printed and scanned labs. Only the Quest labs can be accessed because those are "E" and go direct into AC very nicely. But if the patients use any of the other labs those are paper or fax and they are stuck in dysfunctional FAP for the moment, what can I say? So here is this big "B" and I do mean with a capital B mind you threatening us even though she was given a legit reason as to why we can't give her what she wants. And the interesting thing here really is, that the carrier paid for the labs and that the lab is another "private contractor" that they have a contract with, and so why doesn't she simply go get her labs from the other provider who actually ran the tests in the first place... They too should have the records to share and keep safe for years to come and there is the answer and satisfaction of her request which I was not aware of at the time of the call I fielded... What a moron, what a PITA.

And I think had her tone been more agreeable and less I'm the big "B" who is going to threaten you like some full of themselves middle school teacher, I would not have jumped down her throat so bad... But it was almost like "This is going on your permenent record Mr !" kind of control "B" attitude as though it has come to this that doctors have to kiss the butt and be scared of some idiot, middle nobody threatening them like this because their software is having some delays and difficulties. .. As though their studip little pay poorly for FHP and CHIP plan shouldn't be estatic to have a solo doc who actually see her patients for 20-40 minutes at a clip and uses a (GASP!!!!) CCHIT certified EMR no less... GIve me an freakin' break.... I was shaking by the time I got off the phone with this women, really I was... I just was not going to be intimidated and pushed around by someone who shouldn't really even have a job in the first place. As they said in the Bergus Merideth Twilight Zone when he was a Librarian... "You are Obsolete". These care managment idiots, they are Obsolete....

I forego the plastic holders.  They add to

postage and tend to break/crack anyways.  I fold the instructions for reading

the CD (really everything is in PDF format, but there are some non-computer

literate people out there!) in half, insert the CD and staple once on each

short side and twice on the long side.  Insert into bubble 4x6 envelope and

stick it in the mail.  No one has said that they have been broken, unreadable,

etc.

rigid holder around the CD.

Actually the pt I was going to send off the CD for is coming

back to me as a pt..........

I'll be using this method in the future, however, so am

Wow! I was just reading the first few pages of this guy's book

and it is so clear and simple & yet at the same time so hard, so tough to

find 500 to 750, perhaps as many as a 1,000 patients who so get it, and have

the means to pay for it so as to return to this so needed, lost and wonderful

possibility that in one way or another we here are all attempting to reach.

Some more successfully than others. He just lays it out so nicely and

cleanly. It so simple, so obvious...

I just had some rude women call the office and she

was leaving a message about how she was going to note and not

cooperating with the quality review requesting charts and the like, typical

managed care, FHP's and CHIP's BS. And so I picked her up in mid-sentence and

almost took her head off I was so pissed off while listening to her while at

the same time almost in tears as I was getting things ready for the main

weekly bank run. She was so suprised to be treated so agressively by me, and

I said, " yeah, because I'm not just another leech living off some over

worked under paid doctor, I'm the freakin' doctor's husband and me and my

kids have to live with the results of companies like yours. I gave her gruff

about all the uncompensated BS paperwork and why should some bozo with a BA

hold power of authorization over the choices made by my wife who has three

times the training. I asked just what do you think she studied and

learned in Medical

School????

ANd then I answered my own question with: " She learned how to managed

patients' care, how to care for sick people and take good care of them and

their diseases that's what... " And then I asked who should be the actual

judge of the quality of the doctor's work? Like shouldn't it be the patient's

choice as whether or not the doctor is providing them with good care or not?

Are they not the actual consumer of my wive's services not your company?

She went on about how I was the first

person to challenge her on such things and I should have told Yeah but I

won't be the last that's for sure but it didn't come to me then on the phone.
