Guest guest
The goal is to create a clinically integrated group capable of delivering highly effective/high quality primary care. Any willing player invited, starting with the NYC market. The hope is that this clinically integrated group would attract self-funded employers to pay some sort of extra fee to the practices that would make it possible for the group to succeed in getting great results.I'm proposing that we use HYH as the quality integrating IT engine. We need to clear some legal hurdles, so stay tuned as I talk to lawyers. We should easily avoid anti-trust accusations given the true clinical integration, the question is how to avoid being perceived as an insurance company. Stay tuned.G Is this something to do with Hello Health or is this an IMP group project? Just FYI I'm putting my thoughts to blog in case any want to follow along. Guest guest
Carolyn HYH is HowsYourHealth.org an internet based tool, a patient reported data stream ,that has been shown to be clinically correlated with objective dataHYH is the tool IMPS have been using for 5 or so yrs to measure their work IT is not the most slick site and sometimes we get feedback form the sophisticated that it is clunky Still it is what we use,as at the moment no other tool works like hyhTranslation - patients go to HYH using a passcode that is YOURS and spends 15 min doing a survey or heath check up YOU can use this now yourself free as an individual to try it out . HowsYourHealth.org ; if you as a doc want all the replies to come to you you sign up This is patient reported data It turns out that patients actually KNOW what is going on and it turns out that the measures used on HYHl ike asking about confidence pain money support, when if addressed are a bit of magic that transforms the noncompliant into the heathy get what they need confidenct person PAIn nd poverty and lack of support are tough to counter but can be addressed and at least support offered Chart audits show correlation of patietn entered data to objective. Translation- JOe says in his HYH survey that his bs is uncontrolled and his chart audit agees. HYH also measures things that people get the importance of-- access, efficiency who is in chargeRead a short interesing article by LAcey et all in ANnals of FP a few yrs ago titled Why we don;t come Patients miss out on health care when they can't find a parking spot or cannot get the stroller in the door or have to punch 1 for refills 2 for an appointment today 3 for susie dr x's nurse, 4 for billing etc becasue they don;t know who their doc is anyway can;t hear on the phone and have a question the phone tree doesnt get atThis is the stuff IMPS get at.HYH measures it.:)Really that was way too much typing Come to Camp Denver OCt 2 and 3. Guest guest
That sounds promising Gordon. We'd be interested in joining when you expand beyond NYC. Dr Fine from HealthAccessRI said that they were going to market primary care directly to employers and town governments when he gave a talk here in Maine last year. Some of the RI docs here might know how they've done with that since then. Carolyn, HYH = How's Your Health HYH? On Mon, Mar 1, 2010 at 6:57 PM, L.
