Here is some information I thought I would pass on, concerning additional

Medicare prescription drug cost.

This is a web page for Medicare, www.ssa.gov/ medicareoutreach 2 the form you

want to fill out is form SSA-1020. If you are denied for whatever reason it

helps to appeal the decison and go back to the same web site and fill out an

appeal form.
