Research Study Due Date

Hi all,

I discovered in the process of sending out the questionnaires I did not put a

return date.

I need them back no later than Sept. 23rd, 2010. This gives us 2 weeks to gather

all the information we need to submit for the final stage on Oct. 1st, 2010.

Thank you for your participation, hoping this will change the way patients get

diagnosed and treated.
