We are asking for any doctors that you have been to or see to add to our

Doctor's Map and List. This can be a doctor that you either had a good or bad

experience with. What I will do is add them to the map and list and then add

your contact information, if you wish, so that people can then make a better

informed decision to see a doctor. Send all of the information to me and I will

add them as I can.
