This site has all the forms and manuals to download for the WHO Quality of Life tests.     http://www.psychiatry.unimelb.edu.au/qol/whoqol/whoqol-insruments.html\

I couldn't immediately find all these on the WHO site - just the brief version of the test.WHOQoL Instruments downloads

WHOQoL-100

The

WHOQoL-100 assesses individuals' perception of their position in life

in the context of the culture and value systems in which they live and

in relation to their goals, expectations, standards and concerns.

WHOQoL-Bréf

An

abbreviated 26-item instrument, the WHOQoL-Bréf has been developed.

This shorter version is more convenient for researchers who are

conducting large research studies or clinical trials.

WHOQol-8

The

WHOQol-8 contains eight items that were empirically derived from the

WHOQOL-Bref. As yet no scoring algorithm has been published, although a

summative model is most likely, producing an index. The other

possibility is that each item is scored separately.

CA-WHOQoL-100 (currently unavailable)

The

CA-WHOQoL-100 comprises a translated version of each WHOQoL-100 item,

plus additional items relevant to the Chinese-Australian community. The

response scales for all items were derived specifically for this

instrument using a Chinese-Australian sample.

WHOQoL-OLD (currently unavailable)

The

WHOQoL-OLD is a new instrument that is currently being developed and

will become available in the near future. If you would like more

information about this instrument, please contact the Australian WHOQoL

Field Centre.

WHOQoL-AA (currently unavailable)

The

WHOQoL Attitudes to Ageing is currently being developed and will become

available in the near future. If you would like more information about

this instrument, please contact the Australian WHOQoL Field Centre.

Australian Population Norms

The

Australian population norms are currently being developed as a separate

reference file for your convenience. In the meanwhile, for the

WHOQoL-Bréf, please refer to page 26 of the User Manual and for the

WHOQoL-100, please refer to page 16 of the User Manual.

User Manual

The WHOQoL User manual and interpretation guide,

including the Australian Population Norms. It facilitates

administration, scoring and interpretation of the Australian versions

of the WHOQOL-100 and the WHOQOL-Bréf. It contains information

regarding the development of the WHOQOL instruments, both at an

international and national level, details of the psychometric

properties of the instruments, and guidelines for administration,

scoring and interpretation, together with copies of the Australian

WHOQOL instruments. It is specific to the Australian versions of the

WHOQOL, including both the English versions (the Australian WHOQOL-100

and the Australian WHOQOL-Bréf) and Chinese-Australian version (the

CA-WHOQOL-100). Locke, MD
