Your kind heart may eventually let someone pay later so also consider asking for:

place of employment

Driver's License

SS #

I ask for SSN as most insurance plans want it.

Have not tried it w/o it. Sharon or , do your folks get and money back

from their insurance company if they don’t put their SSN on their CMS 1500?

With bouncing checks, in my county (maybe even state – CO) you

have to have a copy of their drivers license to even consider trying to get the

money from them with the legal system/police.

I've never put a social security number on any of the electronic billing I submit for patients (except Tricare) and it has never been a problem.

Don't forget medicare needing a SS since that is the #. Otherwise I don't put them on, I keep them, if I have them, since some insurers can find a patient this way, if the card number is wrong

True about Medicare.  I'm just currently opted out, so it's not an issue for me.

Don't forget medicare needing a SS since that is the #. Otherwise I don't put them on, I keep them, if I have them, since some insurers can find a patient this way, if the card number is wrong
