As I've said many times before, we can have the patient as ready as we like,

but unless the physician writes the d/c or transfer orders, we have only

increased our costs. Repeatedly, staff indicate the patient is ready to go,

but the physician keeps them longer because " the family is not ready to take

them " , the patient expected to stay longer. " , etc.

With a 3.5 los for some of your ortho, are they going directly home? My

understanding is that they have to stay at least 4.9 days on acute, before

they can be transferred to skilled or rehab, otherwise, you aren't allowed

the full DRG acute payment. (Don't want facilities " double dipping " , i.e,

getting full DRG for short LOS, then transferring to other unit where per

diem rate provided.
