No, I haven't seen one. But, someone on an Alzheimer's list (where Geri Hall

is) said to use something simple.

Please excuse her/him.

She/he has dementia.

Two lines are plenty. This way the print can be large enough that the wait

person/clerk can glance at it in a hurry and stick it in a pocket.
