my IMP less than 1 week old, and need your help please

Dear all,Thank you for the great support and kind help from so many of you, I finally made the leap.My IMP opened on Jan 6!We saw 8 (1/2 day), 12 (full day). 7 (1/2 day) first three days. 2-3 brand-new patients a day and others came from my previous practice. 12 so far on schedule for tomorrow. All new systems, pretty hectic for me and 2 my assistants - one admin + billing (learning to bill); the other is a pre-medical intern- a new CNA graduate who wants to apply for med school later. No pay position, more like work study, so I teach her medical skills and knowledge and she helps with clinical side-working out well for both of us.Busy, but really happy to have the freedom. Still a lot catch up work to do since we took a week off to spend with our children without doing any work at all during Christmas-it was great!Please help me with two urgent issues1. billing- when patients have Medicare as primary and BCBS as secondary which I participate in both, should we charge just the BCBS copay amount, or should we charge the patient directly up to $135 deductible before the secondary will kick in?2. supplies- Where is the best (cheapest) place to buy standard battery for the portable handheld WelchAllyn otoscope?
