RE: CMC: 1187949 FIN# 29953775 DOB# 04/08/02 DOV# 10/02/02 is a 6-month-old little girl who was delivered six weeks premature. She spent a week in the nursery. Shas done well, and there have been problems. She smiled at 2 months, rolled over both directions at 4 months, and has been pulling herself up to sit for the past month or so. There has been no irritability, lethargy, or vomiting. Nor eye-crossing. She has frontal bossin and a narrow forehead. On examination, the head circumference measurement is 42.0 cm, and this plots out at the 60th percentile. This is in comparison to a 50th percentile length and a 25th percentile weight measurement. The child is bright and alert. The extraocular movents are full. The fontanel is soft. The child has good head contrl. There is no weakness or tone abnormality in both upper and lower extremities. The deep tendo reflexes are 1+ and symmetrical thoughout. she has a tiny patch of hemangiomatous skin discoloration over the lumbosacral spine. The head is elongated, and this MAY reflect her prematurity. She has quite a prominent frontal bossing. However, there is a metopic ridge that extends from the nasion up to the hairline, and this is not visible by visual inspection alone. It is palpable, however. The has some bitemporal pinching, and this may be the porduct of her metopic suture fusion or the narrow head shape of a premature. We think that she has a significant cranial deformity, but it is not clear that this will require surgical correction. We are going to try a bi-cal headband to see if this can diminish the forehead prominence. She will also have skull x-rays. We will see her back once this has been completed.
