The link below to my website is for the letters that i used to win the appeal to Blue Cross to cover my buffalo hump in my 2001 surgery. The top is my letter and the bottom is my doc's.

They covered my 2006 liposuction on my hump on the first submittal.

Remember this is for the hump, but you might use the format and some of the wording for your face. Hopefully, you took "before" pics. Also my plan is the same plan for congress (da fed's) so it covers much more than for regular civilians (you don't think congress would stoop to use the same coverage as civilians, do you [calm down, george, life sucks] <g>).
