My Oncologist/HIV doctor has arranged for a local doctor here who has successfully worked with Sculptra to do my work, gratis, just with my Medicaid. Which is astounding. The physician's fee was what I worried about most of all...specialists are not cheap, I know I don't have to tell you all that. But I knew I qualified for the Sculptra product. My Oncologist/HIV doctor suggested that I fill out the application as completely as I could, COMPLETELY. There are just a few fields I cannot fill in (ie DEA number, Sculptra Facility #, etc). He says since this specialist has agreed to do my work without charging me his regular fee, I need to make it as simple as I can for the office to fill out what is left on the application, in other words make it as easy as I possibly can for them so they don't have to put any effort into it. My appointment isn't until the latter part of September, but I have everything in the application ready to go...including proof of income from SS, signatures, etc. The envelope is addressed and stamped, ready to go. I'd like to get the process going, and not wait until my first visit to sit and just " talk " with him, although its going to talk a while for the PAP to kick in, I realize, but the time frame would be shorter if I started now. I'd like to drop it by the office and have them fill out his part and send it off. Does that sound pushy. I really don't want to upset the apple cart on anything. Should I just lay back and wait for my appointment to hand him the application, or drop it by the office and have them complete what I cannot complete, and send it off?
