well done Anchita. Here in the UK we have RCOG , NICE and local hospital

guidelines and protocols which are very useful. Yes Misoprostol 800

microgm, PR is cheap and effective method of controlling/preventing PPH

Dear All,

Monday, i.e. the 11th of April was the National Safe Motherhood Day.

Apart from the obvious national significance, and the importance in

every doctor's life, esp. Ob-Gyn nad Public Health specialists, this

year the day assumed great importance for me profesionally.

As I'd written before, I am working as a Consultant to the Ministry of

Health, the Maternal Health Divison. One of my " deliverables " (excuse

the job lingo) was a technical protocol titled " Guidelines for

Ante-natal Care and Skilled attendance at birth by ANMs and LHVs " . This

" book " was officially released at the hands of the Secretary Health and

family Welfare on Monday. As the " chief author " you can imagine my pride

on seeing my book released with the GoI cover and under the GoI banner

and at the hands of an Impt person in the " system " .

Incidentally, these guidelines are a landmark of sorts, as they bring to

the nursing cadre certain permisiions for things that they were not

allowed to do earlier (officially). For e.g. believe it or not, till now

the nurse was not allowed to give an IV (Inj. or fluids)!!! But here the

group at the Ministry (with me as the youngest member) went back to the

Drug Controller of India to seek modifications in the rules, and got

approval. Other approvals include the use of oral Misoprostol ( a

uterotonic drug, sometimes used for Medical abortion) routinely after

each delivery for the prevention of PPH etc.(Any comments Kishoreda, as

a Gyneacologist?)

To be part of such " historic changes " feels good. And of course the

professional appreciation and recognition is a much welcome " fallout " of

this endevour. Very soon similar guidelines will be released for the

Medical officers (MBBS) too. Of course, I am the one writing it again!

But before I take all the credit, i must say that there have been people

here in the Ministry and with the UN agencies (incl WHO) who have gone

out of their way to help me, and give me technical inputs wherever

required. I am a public Health person, so you can imagine what i went

thru while writing an Obst. Book!! But there were Numerous Gynaecs more

than willing to help out!

We often blame the govt. fot " sitting idle " . Now that I'm here i can say

that they work. But they too are the victims of red-tapism. They fight

it the best they can. But a serious and sincere officer with a good team

can achieve a lot.

First and foremost, Anchita, let me congratulate you.

Regarding the usage of various drugs by ANMs, I have mixed feelings. Let me

marshal my thoughts and I shall come back to you.
