Guest guest Posted June 22, 2004 Report Share Posted June 22, 2004 Hi, When,I first started using Dorzolamide,way back in 1999,I had seen only 2-3 cases of frank SJ Syndrome,prior to that,during and after my PG.(The first was when we were asked to see a case in the dermatology dept,in Medicine wards,of our college,headed by one Dr.Sharma,AND THE PATIENT WAS A FULL BLOWN CASE,and I as an HO, then would go twice daily,to sweep the fornix with a sterile glass rod to prevent Symblepheron.The patient however died,later due to systemic complications. In 2000,we had one patient and in 2002,we had another,who had no prior history of allergy to Sulfa,and came to us after avg 2weeks of using monotherapy from outside,with frank inferior conjuctival redness,and small blister like lesions,taking me by surprise.Then,my fellow senior consultant,decided to stop the drug,call over the local representative and seek their views.They photographed,had it sent to their company and their panel of experts from US,and,I think,also from Switzerland reviewed and said,probaly,it was A. Preservative induced bullous pemphigoid ,early stage B. Preservative induced Toxic conjunctivitis. Blister reports were normal.Patient recovered after 15days,with mild fibrosis of inferior conjunctiva. The second had a less severe reaction.In both cases,we felt it was early SJ,contrary to what they made us feel. I still use it in cases after laser iridotomy,ALT,and it gives me good results,without BETA-BLOCKER,induced side effects. But the Cipla people chose the wrong day, Saturday, because most of the private consultants here do not get over their clinics till well after 9 PM. So the program started at around 9:45 PM. We had our JDOS session first, followed by the Cipla product launch session, the two together took about 45 minutes... It was a cocktail dinner, Hotel Satya Ashoka is an excellent place, exhaustive cuisine, etc. We have been using Dorzolamide in the form of Cosopt/Trusopt (MNC's) for the last 3 years, but not as a stand alone (monotherapy), that is what Cipla have launched in the form of Dorzox. Personally I feel Dorzolamide 2% eye drops in the form of monotherapy is valueless (since the pressure reduction lies between that of timolol and betaxolol). In the launch literature I read that no cases of SJ syndrome have been reported so far (world-wide), so I am surprised that you came across two. We have not had any experiences so far, at least with adjunctive dorzolamide. But now, with the launch of stand-alone dorzolamide, lets see!
