I will ponder your problem and try to come up with some ideas, but currently have the flu and am not feeling very sharp (at all). What I did want to suggest is that you could possible fill an Ensure can or bottle with water for your older daughter, if she would agree to participate. You'd have to be pretty sneaky, but if it got your younger girl to drink it, it would be worth it. I'm sure your older daughter doesn't want to gain weight, so that makes everything doubly tough. One kind of exposure I could imagine, which sounds pretty harsh, might be to have your older daughter report what she ate, and after your younger daughter eats it, she could say, " actually I didn't eat that at all. " After the tantrums settle down, it might show your younger daughter that nothing horrible happens when she eats something different. Again, you'd have to enlist the help of the big sister.
