Are you doing free weights, or are you using nautilus-type machines? One thing that helps me is to write down what I do each time. And even if I can't get through all 12 reps at a higher weight when I'm trying to increase the weight, I try to at least get through 6 and work my way up over a few times. Especially with free weights, form is really important so that might be something to consider. If you have someone who can critique your form while you're lifting they might be able to give you some IRL feedback on that.
