charging pt a voluntary fee of $50 a year to offset some of the costs of all

these unreimbursed care. Whoever that was, could you respond to me off or

on list. I am now thinking about doing the same thing.’

I’m not sure who ‘magnetdoctor’ is, but

you may have been thinking of me.

Since I opened in 7/03, I have charged an ‘Annual Fee’

for this unreimbursed work: communication via email/fax/phone, virtual

office visits, filing out simple forms (camp physicals, e.g.) lab results w/o

appt, med refills w/o an appt, etc if clinically appropriate. This is not

an optional fee – it is required to be in my practice, and a few (and I

mean ~10 people total that I am aware of since I opened) have not joined my

practice because of it. It was $50 for the first 2 years, then $60 for

2006 (with $20 additional per virtual office visit). These fees have been

paid by Kaiser for their members as they are studying my practice. I have

yet to figure out the best way to do this, and am working on how to do it for

2007. I’m trying to get away from a set fee/person/year and

approach a more cost-per-use basis.

There was someone else, in Florida I believe, who did have an ‘optional’

fee – not a required fee. Can’t remember who it was,

but I thought it was a male doctor, and it was higher than $50.
