becoming more efficient

and now, the latest update-- i've made the plunge, upgrading from dragon naturally speaking preferred 7 to pro 8, and from a logitech desktop microphone to a 900 mhz wireless transmitter and receiver, with a headset microphone. i am no longer tethered to my desk. the microphone headband does not cover my ears, so i can wear it and still use my stethoscope. the microphone plugs into a transmitter the size of a pack of cigarets, which either clips onto my belt or sits in my shirt pocket. the receiver plugs into the onboard sound card of the computer motherboard. with relatively little training, i have easily 95% accuracy or better. what a difference! i am used to dictating from internship, and i feel that speaking is the fastest form of communication, especially when using voice macros. in addition, one can also use the voice recognition commands for control of the computer

and software. i am using AC, and the dictation works great. i will have to make some custom commands for AC, but hope to soon be able to manipulate the entire emr using voice. now, i am truly able to complete the entire note in the presence of the patient, who hears what i am dictating, and who can confirm the accuracy of my chart note. Nice work Larry. That is on my wish list for the new office to open in

March. Will be getting VR set up... hopefully from day one... and would

love to get to the point you are describing asap. I may be sending you

some email questions off the loop if I need pointers along the way.

....and my latest good news is that the lease is signed, insurance set, I

have keys and the builders started putting up the studs for a new wall and

will be knocking out part of another wall for a new door soon. I'm taking

pictures along the way and will send out a link to them in a couple weeks

as the office takes shape.

first, i don't believe dragon pro medical is necessary; like most other things sold to doctors, the price is just jacked up. i purchased my upgrade to dragon 8 pro from knowbrainer, and they have their own set of medical vocabularies, in addition to command software, which i also bought. second, i believe dragon is a user license rather than a site license, so you should be able to install it on as many computers you want and use, but only you can use it. if there are other users, then i believe you would have to purchase additional user licenses. third, there are lots and lots of microphones and microphone systems out there, the best being the wireless sennheiser system, which retails for about $600-800, uses a headset microphone similar to mine, and has autoscanning uhf to find the clearest channel. i initially tried a very low power fm system from radio shack for $25, which i then modified to accept a radio shack headset

microphone, a sennheiser knock-off. unfortunately, there's tremendous fm interference in my area since i am in direct line of sight from a mountaintop with tons of rf equipment, but it does work @ home. since i use a 900 mhz cordless telephone @ work, i figured a similar system was worth a try. unfortunately, radio shack had discontinued their 900 mhz audiolink transmitter, but i found one on ebay for $10 new. i got it and their 900 mhz receiver (yes, the 8 programmable frequencies match), plugged in my radio shack sennheiser headset microphone knock-off, and away i went. works great. the bad news is that radio shack has discontinued their headset microphone, however there may still be some in stores, and knowbrainer has some, too, i think. it's part # 33-3012, and the clearance price is $30. the audiolink transmitter is again in stores, as is the receiver, part #'s 32-1252 and 32-1250, $50 and $80

respectively, or they come together with a handheld microphone. i don't know the model # of that combination. my total cost for the wireless microphone system was $30 + 10 + 70 = 110, plus shipping. i think the system works well, and is a great deal, even at full price. another important thing is that the cheap logitech desktop microphone i was using works well with dragon 8. the only modification to it i made was when i started using dragon 7 preferred with it; i added a taped paper cone around the stem and put a piece of closed cell foam into the cone. hope that's helpful.
